细节护理在颌面部皮瓣修复中的干预效果

Intervention effect of detail nursing in maxillofacial flap reconstruction

目的探讨细节护理在手术室口腔颌面部游离皮瓣修复手术患者中应用的效果。方法选取2019年5月—2021年5月64例在天津市口腔医院进行口腔颌面部游离皮瓣修复手术的患者作为本次研究对象,通过随机摸球的方法分为对照组和研究组各32例。对照组实施常规护理(一般护理、心理护理、口腔护理、饮食护理),研究组实施细节护理(等待区护理、术前护理、皮肤护理、术中护理、术后护理)。比较两组患者在不同护理方式下的护理效果。结果两组随着时间的延长VAS评分均降低,研究组患者VAS评分低于对照组,差异有统计学意义(P<0.05)。研究组患者住院天数(21.75±5.05)d,短于对照组的(28.40±5.38)d,血肿发生率为9.38%,低于对照组的31.25%;差异均有统计学意义(P<0.05);研究组患者皮瓣局部坏死发生率12.50%,低于对照组的25.00%,差异无统计学意义(P>0.05)。结论细节护理在手术室口腔颌面部游离皮瓣修复手术患者中应用能减轻患者术后疼痛,缩短住院时间,降低血肿发生率,值得临床推广应用。

Objective To explore the effect of detail nursing in patients undergoing oral and maxillofacial free flap reconstruction in operating room.Methods Sixty-four patients who underwent oral and maxillofacial free flap reconstruction in Tianjin Stomatological Hospital from May 2019 to May 2021 were selected as the subjects of this study,and divided into control group and study group with 32 cases in each group by randomly touching the ball.The control group were given routine nursing(general nursing,psychological nursing,oral nursing and dietary nursing),and the study group were given detailed nursing(waiting area nursing,preoperative nursing,skin nursing,intraoperative nursing and postoperative nursing).The nursing effects of the two groups under different nursing methods were compared.Results The VAS score of the two groups decreased with the extension of time,and the VAS score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The length of stay in the study group was(21.75±5.05)d,which was shorter than that in the control group with(28.40±5.38)d;the incidence of hematoma in the study group was 9.38%,which was lower than 31.25%in the control group;and the differences were statistically significant(P<0.05).The incidence of local flap necrosis in the study group was 12.50%,which was lower than 25.00%in the control group,and the difference was not statistically significant(P>0.05).Conclusion The application of detailed nursing in patients with oral and maxillofacial free flap repair in the operating room could reduce postoperative pain,shorten hospital stay,and reduce the incidence of hematoma,which was worthy of clinical promotion and application.
