脑清喷鼻微乳对急性缺血性脑卒中言语不利患者的影响

Effect of Naoqing Nasal Spray Microemulsion on Patients with Speech Impairment in Acute Ischemic Stroke

【目的】评价脑清喷鼻微乳(主要成分为麝香、冰片、川芎、石菖蒲、三七、薄荷等)对急性缺血性脑卒中(acute ischemic stroke,AIS)患者言语功能障碍的影响。【方法】将60例就诊于广州医科大学附属中医医院且明确诊断为AIS伴言语不利的患者随机分为试验组和对照组,每组各30例。2组患者均给予西医常规治疗,在此基础上,试验组给予脑清喷鼻微乳喷鼻治疗,对照组给予空白喷鼻安慰剂治疗,疗程为10 d。观察2组患者治疗前后中医证候积分、美国国立卫生研究院卒中量表(NIHSS)评分、改良Frenchay构音障碍评估量表(FDA)评分、Barthel指数评分的变化情况,并评价2组患者的临床疗效。【结果】(1)治疗10 d后,试验组的总有效率为93.33%(28/30),对照组为56.67%(17/30),组间比较(χ^(2)检验),试验组的疗效明显优于对照组(P<0.01)。(2)治疗后,2组患者的NIHSS评分和中医证候积分均较治疗前降低(P<0.05或P<0.01),且试验组的降低幅度均明显优于对照组(P<0.05)。(3)治疗后,2组患者的FDA评分、Barthel指数评分均较治疗前升高(P<0.05或P<0.01),且试验组的升高幅度均明显优于对照组(P<0.01)。【结论】脑清喷鼻微乳能有效改善AIS患者的言语功能,促进神经功能恢复,改善患者的中医证候,提高患者的日常生活自理能力。

Objective To evaluate the effect of Naoqing Nasal Spray Microemulsion(the main ingredients including Moschus,Borneolum Syntheticum,Chuanxiong Rhizoma,Acori Tatarinowii Rhizoma,Notoginseng Radix et Rhizomam,and Menthae Haplocalycis Herba)on the speech impairment of patients with acute ischemic stroke(AIS).Methods Sixty patients who were diagnosed as AIS with speech impairment in the Affiliated Traditional Chinese Medicine Hospital of Guangzhou Medical University were randomly divided into a trial group and a control group,with 30 patients in each group.Both groups were given conventional western medical treatment,and additionally the trial group was given Naoqing Nasal Spray Microemulsion while the control group was given the blank nasal spray as the placebo.The course of treatment for the two groups covered 10 days.The changes of traditional Chinese medicine(TCM)syndrome score,the National Institutes of Health Stroke Scale(NIHSS)score,the modified Frenchay Dysarthria Assessment Scale(FDA)score,and the Barthel Index score in the two groups were observed before and after treatment.Moreover,the clinical efficacy of the patients in the two groups was evaluated.Results(1)After 10 days of treatment,the total effective rate of the trial group was 93.33%(28/30)and that of the control group was 56.67%(17/30),and the intergroup comparison(tested by chi-square test)showed that the efficacy of the trial group was significantly superior to that of the control group(P<0.01).(2)After treatment,the NIHSS scores and TCM syndrome scores in the two groups were lower than those before treatment(P<0.05 or P<0.01),and the decrease of the scores in the trial group was significantly superior to that in the control group(P<0.05).(3)After treatment,the FDA score and Barthel Index score of the two groups were increased compared with those before treatment(P<0.05 or P<0.01),and the degree of the increase in the trial group was significantly superior to that in the control group(P<0.01).Conclusion Naoqing Nasal Spray Microemulsion can effectively improve the speech function of AIS patients,promote the recovery of neurological function,relieve the TCM syndromes,and enhance the abilities of daily life of the patients.
