杨祥正治疗儿童咳嗽变异性哮喘用药规律分析

Analysis of Medication Rules of YANG Xiangzheng in Treating Cough Variant Asthma in Children

目的:挖掘杨祥正教授治疗小儿咳嗽变异性哮喘的用药规律,为中医儿科临床治疗咳嗽变异性哮喘提供诊疗依据。方法:收集2020年8月—2021年8月杨祥正教授治疗咳嗽的验案验方,录入MicrosoftExcel及中医传承辅助平台(V2.5),统计药物频次;运用Apriori算法及关联规则对处方核心组合进行统计分析(支持度为0.6,置信度≥0.95);根据无监督的熵层次聚类法提取内在核心组合和新方组合(相关度为6,惩罚度为2)。结果:共收集整理得217首处方,涉及143味中药,其中用药频次较高的有甘草、茯苓、百部、五味子、杏仁、橘红等;常用药物组合有甘草+茯苓,甘草+百部,茯苓+五味子,茯苓+百部,甘草+五味子等;并挖掘出内在核心组合10个以及新方5首。结论:杨祥正教授针对于小儿咳嗽变异性哮喘以化痰止咳平喘为治疗大法,由内在关联聚类而成的5首新方,分别应用润肺敛阴止咳、清热化痰、疏散风热、祛湿化痰,温阳散饮等法,可对临床治疗小儿咳嗽变异性哮喘提供参考。

Objective:To explore the medication rules of Professor YANG Xiangzheng in treating cough variant asthma in children,and provide diagnostic and therapeutic basis for the clinical treatment of cough variant asthma in Chinese medicine pediatrics.Methods:Professor YANG Xiangzheng's proven prescriptions for treating cough from August 2020 to August 2021 were collected and input into Microsoft Excel and the Traditional Chinese Medicine Inheritance Support System(V2.5)to calculate the frequency of Chinese medicines;statistical analysis of core combinations in those prescriptions was carried out by using Apriori algorithm and association rules(the support level was 0.6 and the confidence level was greater than or equal to 0.95);core combinations and new prescriptions and combinations were extracted based on unsupervised entropy hierarchical clustering method(the correlation degree was 6 and the penalty degree was 2).Results:A total of 217 prescriptions were collected and organized,involving 143 Chinese medicines.Among them,Glycyrrhizae Radix Et Rhizoma,Poria,Stemonae Radix,Schisandrae Chinensis Fructus,Armeniacae Semen Amarum,Citri Exocarpium Rubrum were Chinese medicines with higher using frequency.The common medicinal combinations were Glycyrrhizae Radix Et Rhizoma and Poria,Glycyrrhizae Radix Et Rhizoma and Stemonae Radix,Poria and Schisandrae Chinensis Fructus,Poria and Stemonae Radix,Glycyrrhizae Radix Et Rhizoma and Schisandrae Chinensis Fructus.A total of 10 core combinations and 5 new prescriptions were excavated.Conclusion:In the treatment of cough variant asthma in children,Professor YANG Xiangzheng takes dissolving phlegm and relieving cough and panting as the main treatment method;the five new prescriptions formed by internal correlation clustering respectively apply the methods of moistening the lung and constraining yin to stop cough,clearing heat and resolving phlegm,scattering wind and discharging heat,dispelling dampness and resolving phlegm,and warming yang and dissolving fluid retention,which can provide a reference for clinical treatment of cough variant asthma in children.
