糖化血红蛋白联合血清糖类抗原19-9对2型糖尿病的诊断价值

Diagnostic value of combined detection of HbA1c and serum CA19-9 in type 2 diabetes

目的探讨糖化血红蛋白(HbA1c)与血清糖类抗原19-9(CA19-9)联合检测在2型糖尿病(T2DM)中的诊断价值。方法选取2021年1月至2023年1月在漯河医学高等专科学校第二附属医院就诊的T2DM患者89例作为T2DM组,并以1∶1配比选取同期健康体检者89名作为健康组,均检测HbA1c、CA19-9水平,对比2组HbA1c、CA19-9水平、有无血管病变T2DM患者HbA1c、CA19-9水平,统计不同HbA1c水平、CA19-9阳性和阴性T2DM患者并发症情况,分析HbA1c水平与CA19-9的关系。结果T2DM组HbA1c、CA19-9高于健康组(P<0.05);与无血管病变相比,有血管病变T2DM患者HbA1c、CA19-9水平较高(P<0.05);与HbA1c水平<7.5%、7.5~9.5%并发症发生率15%(2/13)、37%(10/27)相比,HbA1c>9.5%并发症发生率65%(32/49)较高(P<0.05);与CA19-9阴性并发症发生率13%(4/31)相比,阳性T2DM并发症发生率52%(30/58)较高(P<0.05);随着HbA1c升高,CA19-9阳性率升高,阴性率降低(P<0.05)。结论HbA1c、CA19-9参与T2DM发生,与疾病发展联系紧密,二者联合检测可提高诊断效率,对预防T2DM并发症具有积极作用。

Objective To explore the diagnostic value of combined detection of HbA1c and serum sugar antigen 19-9(CA19-9)in type 2 diabetes(T2DM).Methods A total of 89 T2DM patients who were admitted to the Second Affiliated Hospital of Medical College from January 2021 to January 2023 were selected as T2DM group,and 89 healthy individuals who underwent physical examinations during the same period were selected as the health group in a 1:1 ratio.HbA1c and CA19-9 levels were detected in both groups,and HbA1c and CA19-9 levels in T2DM patients with and without vascular disease were compared between the two groups.The complications of different HbA1c and CA19-9 positive and negative T2DM patients were statistically analyzed between HbA1c level and CA19-9.Results The levels of HbA1c and CA19-9 in T2DM group were higher than those in healthy group(P<0.05).Compared with those without vascular disease,the levels of HbA1c and CA19-9 in T2DM patients with vascular disease were higher(P<0.05).When the HbA1c level is less than 7.5%or 7.5%~9.5%,the incidence of complications is 15%(2/13)and 37%(10/27),respectively;Compared to that,when HbA1c>9.5%,the incidence of complications was higher(65%,32/49)(P<0.05).With the increase of HbA1c,the positive rate of CA19-9 increased,and the negative rate decreased(P<0.05).Conclusion HbA1c and CA19-9 are involved in the occurrence of T2DM and are closely related to disease development.The combined detection of HbA1c and CA19-9 could improve diagnostic efficiency and have a positive effect on preventing complications of T2DM.
