1206名社区居民肺通气功能状况及影响因素分析

Analysis of the current status and influencing factors of lung ventilation function among 1206community residents

摘要 目的了解深圳市龙岗区南联社区居民肺通气功能状况及其影响因素,为基层医疗机构开展慢性呼吸系统疾病的防治工作提供参考依据。方法选取2021-01-01-2021-12-31接受深圳市龙岗中心医院南联社区健康服务中心肺通气功能检查的1206名居民作为研究对象,采用自制问卷调查肺通气功能相关影响因素,计数资料的单因素分析使用χ2检验,多因素分析使用logistic回归。结果问卷完整且肺通气功能检查质控合格的居民共1206名,肺通气功能正常788名(65.34%),肺通气功能检查结果异常418名(34.66%)。单因素分析结果表明,性别(χ^(2)=32.195,P<0.001)、年龄(χ^(2)=8.858,P=0.012)、体质量指数(BMI,χ^(2)=10.900,P=0.004)、是否吸烟(χ^(2)=49.876,P=0.005)、是否饮酒(χ^(2)=9.802,P<0.001)和是否坚持锻炼(χ^(2)=244.415,P<0.001)与肺通气功能异常发生率有关;多因素分析结果表明,男性(OR=1.429,95%CI为1.005~2.030,P=0.047)和吸烟(OR=2.132,95%CI为1.482~3.069,P<0.001)是肺通气功能异常的危险因素,超BMI(OR=0.652,95%CI为0.491~0.866,P=0.003)和坚持锻炼(OR=0.123,95%CI为0.092~0.164,P<0.001)是肺通气功能异常的保护因素。结论深圳市龙岗区南联社区居民的肺通气功能异常发生率较高,不同特征居民的肺通气功能异常发生率有差异,在临床工作中应引起注意,针对不同特征的居民制定不同的干预措施,达到改变预后效果,降低致残率和减少并发症的目的。 Objective To understand the pulmonary ventilation function and its influencing factors of residents in Nanlian Community in Longgang District of Shenzhen,and provide reference for the prevention and treatment of chronic respiratory diseases in primary medical institutions.Methods Totally 1206residents who received the pulmonary ventilation function examination of Nanlian Community Health Service Center of Shenzhen Longgang Central Hospital from January 1to December 31in 2021were selected as the study subjects.The relevant influencing factors of pulmonary ventilation function were investigated with a self-made questionnaire.Chi-square test was used for univariate analysis of counting data,and logistic regression was used for multivariate analysis.Results A total of 1206residents with complete questionnaires and qualified quality control of pulmonary ventilation function examination,788(65.34%)of them had normal pulmonary ventilation function,and 418(34.66%)had abnormal pulmonary ventilation function examination results.The results of univariate analysis showed that sex(χ^(2)=32.195,P<0.001),age(χ^(2)=8.858,P=0.012),body mass index(BMI,χ^(2)=10.900,P=0.004),smoking(χ^(2)=49.876,P=0.005),drinking(χ^(2)=9.802,P<0.001)and exercise(χ^(2)=244.415,P<0.001)were related to the incidence of abnormal pulmonary ventilation function.The results of multivariate analysis showed that male(OR=1.429,95%CI:1.005-2.030,P=0.047)and smoking(OR=2.132,95%CI:1.482-3.069,P<0.001)were risk factors for abnormal pulmonary ventilation function,while excess BMI(OR=0.652,95%CI:0.491-0.866,P=0.003)and persistent exercise(OR=0.123,95%CI:0.092-0.164,P<0.001)were protective factors for abnormal pulmonary ventilation function.Conclusions The residents of Nanlian Community in Longgang District of Shenzhen have a high rate of abnormal pulmonary ventilation function,and the incidence of abnormal pulmonary ventilation function varies among people with different characteristics.Attention should be paid in clinical work,and different intervention measures should be formulated for people with different characteristics to change the prognosis,reduce the disability rate and reduce complications.
