血清肿瘤标志物癌胚抗原糖链抗原724糖链抗原153联合检测在胃癌诊断中的价值

Combined detection of serum tumor markers carcinoembryonic antigen,glycan antigen 724,and glycan antigen 153 in the diagnosis of gastric cancer

目的探讨血清肿瘤标志物癌胚抗原(CEA)、糖链抗原724(CA724)、糖链抗原153(CA153)联合检测在胃癌诊断中的价值。方法选取2020年1月至2023年1月在江苏省玉山县人民医院就诊的60例胃癌患者为胃癌组,另取同期60名健康体检者为对照组,比较2组受试者CEA、CA724、CA153水平,比较不同分期胃癌患者CEA、CA724、CA153水平,分析CEA、CA724、CA153联合检测在胃癌诊断中的价值。结果胃癌组CEA、CA724、CA153水平分别为(11.2±1.7)ng/ml、(25.4±2.6)U/ml、(50.7±5.8)U/ml,均高于对照组(1.9±0.3)ng/ml、(3.5±0.4)U/ml、(11.8±2.5)U/ml,差异有统计学意义(P<0.05);随着胃癌患者分期升高,CEA、CA724、CA153水平依次升高,两两比较差异有统计学意义(P<0.05);受试者工作特征(ROC)曲线结果显示,CEA诊断胃癌的曲线下面积(AUC)和截点值分别为0.752、6.41 ng/ml,CA724诊断胃癌的AUC和截点值分别为0.809、12.05 U/ml,CA153诊断胃癌的AUC和截点值分别为0.676、37.56 U/ml,三者联合诊断胃癌的AUC为0.924,高于单项诊断,差异有统计学意义(P<0.05)。结论CEA、CA724、CA153联合检测在胃癌诊断中的价值较高。

Objective To explore the value of combined detection of serum tumor markers carcinoembryonic antigen(CEA),carbohydrate antigen 724(CA724),and carbohydrate antigen 153(CA153)in the diagnosis of gastric cancer(GC).Methods 60 GC patients who visited Yushan County Huangjiasi Hospital,Yushan County People′s Hospital from January 2020 to January 2023 were selected as the GC group,and 60 healthy individuals who underwent physical examination during the same period were selected as the control group.The CEA,CA724,and CA153 levels of the two groups of subjects were compared,and the CEA,CA724,and CA153 levels of GC patients at different stages were compared.Results The value of combined detection of CEA,CA724,and CA153 in the diagnosis of GC was analyzed.The levels of CEA,CA724,and CA153 in the GC group were(11.2±1.7)ng/ml,(25.4±2.6)U/ml,and(50.7±5.8)U/ml,respectively,higher than those in the control group(1.9±0.3)ng/ml,(3.5±0.4)U/ml,and(11.8±2.5)U/ml,with statistical significance(P<0.05).As the stage of GC patients increased,the levels of CEA,CA724,and CA153 increase sequentially,with a statistically significant difference between the two groups(P<0.05).The results of the ROC curve showed that the AUC and cutoff values for CEA diagnosis of GC were 0.752 and 6.41 ng/ml,respectively.The AUC and cutoff values for CA724 diagnosis of GC were 0.809 and 12.05 U/ml,respectively.The AUC and cutoff values for CA153 diagnosis of GC were 0.676 and 37.56 U/ml,respectively.The AUC for the combined diagnosis of GC was 0.924,which was higher than that for single diagnosis,and the difference was statistically significant(P<0.05).Conclusion The combined detection of CEA,CA724,and CA153 has high value in the diagnosis of GC.
