国内外过度诊断相关研究的现状及热点分析

Analysis of the Current Status and Hotspots of Domestic and Foreign Research About Overdiagnosis

目的:基于文献计量学方法,分析目前中英文数据库中过度诊断相关研究的现状及热点,为后续研究提供参考。方法:针对核心合集(Web of Science Core Collection,WOSCC)英文数据库和中国知网(CNKI)中文数据库进行文献检索,检索时间为建库起至2023年5月31日。采用CiteSpace 6.2.R4对纳入的过度诊断研究文献进行可视化图谱绘制并解读。结果:共纳入2513篇英文文献和52篇中文文献。中英文文献的年发文量均呈现上升趋势,2010年后英文文献的发文量增速显著。发表过度诊断英文文献最多的国家均为高收入发达国家,且各国间合作紧密;而国内文献的发文机构之间的合作网络相对有限。关键词聚类方面,英文文献主要聚焦乳腺癌、前列腺癌、甲状腺癌、肺癌、心肌病等病种的过度诊断研究;而中文文献的聚类较少且分散,主要为过度治疗、早诊早治与甲状腺癌,研究内容整体呈碎片化。结论:国外的过度诊断研究较丰富且起步较早,而国内研究仍处于发展初级阶段,仍需进一步推进我国相关研究的开展。

Objective:To analyze the current research status and hotspots about overdiagnosis in Chinese and English databases based on bibliometric methods,so as to provide references for subsequent research.Methods:Literature search was conducted in the English database of Web of Science Core Collection(WOSCC)and the Chinese database of China National Knowledge Infrastructure(CNKI),from the establishment of the database to May 31,2023.The CiteSpace 6.2.R4 was used to visualize and interpret the included literature about overdiagnosis.Results:2513 English and 52 Chinese publications were included in this study.The annual publication volume of both Chinese and English literature had shown an increasing trend,and the publication volume of English research had increased significantly since 2010.The countries with the highest number of publications about overdiagnosis in English were all high-income developed countries with strong collaboration between countries,while the cooperation network in Chinese literature was untight.According to keyword clustering,it showed that the English research mainly focused on breast cancer,prostate cancer,thyroid cancer,lung cancer,cardiomyopathy and other diseases about overdiagnosis,while the Chinese literature had fewer and more fragmented clusters,which mainly focused on over-treatment,early diagnosis and treatment,and thyroid cancer,with an overall fragmentation of research content.Conclusion:The research topics of literature in Chinese about overdiagnosis lag significantly behind the research in English.It is suggested that more research about overdiagnosis should be actively promoted and conducted in China in the future.
