摘要
目的 比较缬更昔洛韦与常规抗病毒药物更昔洛韦用于异基因造血干细胞移植(allo-HSCT)后巨细胞病毒(CMV)血症抢先治疗时的成本-效果。方法 选取2015年6月至2022年6月于唐山市华北理工大学附属医院血液科行allo-HSCT且合并CMV血症的患者40例作为研究对象,按治疗方案不同将其分为更昔洛韦组(A组,n=15)和缬更昔洛韦组(B组,n=25)。比较两组临床疗效、成本-效果,并进行敏感性分析。结果 B组患者治疗有效率为88.00%,高于A组80.00%,差异无统计学意义(χ^(2)=0.471,P=0.493)。A组与B组的成本-效果比分别为804.69、121.26元,增量成本-效果比为6 713.01元。结论 缬更昔洛韦相较于更昔洛韦治疗allo-HSCT后CMV血症的临床效果更为确切,可有效提高CMV转阴率,且成本较低,具有更好的经济学效果。
Objective To compare the cost-effectiveness of valganciclovir and ganciclovir in the pre-emptive treatment of cytomegalovirus(CMV)after allogeneic hematopoietic stem cell transplantation(allo-HSCT).Methods Forty patients with allo-HSCT complicated with CMV from June 2015 to June 2022 in the Department of Hematology,Affiliated Hospital of North China University of Science and Technology,Tangshan were retrospectively analyzed and divided into ganciclovir group(group A,n=15)and valganciclovir group(group B,n-25)according to different treatment regiments.The clinical efficacy,cost-effectiveness and sensitivity analysis were compared between the two groups.Results The effective rate of group B was 88.00%,higher than that of group A(80.00%),and the difference was statistically significant(χ^(2)=0.471,P-0.493).The cost-effectiveness ratio of group A and group B 804.69 yuan and 121.26 yuan,respectively,and the incremental cost-effectiveness ratio was 6713.01 yuan.Conclusion Compared with ganciclovir in the treatment of CMV after Allo-HSCT,Valganciclovir has a more accurate clinical effect,can effectively improve the negative conversion rate of CMV,andhas alowercostandbettereconomiceffect.
