器械辅助软组织松解技术在创伤后膝关节僵硬患者康复治疗中的应用效果

Application effect of instrument-assisted soft tissue mobilization in the rehabilitation therapy of patients with post-traumatic knee stiffness

目的观察器械辅助软组织松解技术(IASTM)在创伤后膝关节僵硬(PTKS)患者康复治疗中的应用效果。方法将76例PTKS患者随机分为对照组和观察组,每组38例。对照组接受常规康复治疗,观察组在对照组的基础上联合IASTM治疗。比较两组患者治疗前后的膝关节疼痛视觉模拟量表(VAS)评分、膝关节活动度[最大屈曲角度(ROM-F)、最大伸直角度(ROM-E)、总体活动度(ROM-T)]及膝关节Lysholm评分。结果治疗前,两组患者的膝关节疼痛VAS评分、ROM-F、ROM-E、ROM-T及膝关节Lysholm评分差异无统计学意义(P>0.05);治疗后,两组患者的膝关节疼痛VAS评分、ROM-F、ROM-E、ROM-T及膝关节Lysholm评分较治疗前改善,且观察组的膝关节疼痛VAS评分、ROM-F、ROM-T及膝关节Lysholm评分优于对照组(P<0.05)。结论在常规康复治疗的基础上联合IASTM治疗,可缓解PTKS患者的膝关节疼痛,改善患者膝关节的活动度及功能,其效果优于单纯的常规康复治疗,值得临床应用推广。

Objective To observe the application effect of instrument-assisted soft tissue mobilization(IASTM)in the rehabilitation therapy of patients with post-traumatic knee stiffness(PTKS).Methods A total of 76 patients with PTKS were randomly divided into control group or observation group,with 38 cases in each group.The control group received conventional rehabilitation therapy,based on which the observation group received the combined therapy of IASTM.The pre-and post-treatment Visual Analogue Scale(VAS)score for knee joint pain,range of motion for knee joint(the maximum flexion range of motion[ROM-F],maximum extension range of motion[ROM-E],and total range of motion[ROM-T]),and the Lysholm score for knee joint were compared between patients of the two groups.Results Before treatment,there was no statistically significant difference in the VAS score for knee joint pain,ROM-F,ROM-E,ROM-T,and Lysholm score for knee joint between the two groups(P>0.05).After treatment,the VAS score for knee joint pain,ROM-F,ROM-E,ROM-T,and Lysholm score for knee joint of patients in both groups were ameliorated as compared with before treatment,and the VAS score for knee joint pain,ROM-F,ROM-T,and Lysholm score for knee joint in the observation group were superior to those in the control group(P<0.05).Conclusion Combing with IASTM therapy based on conventional rehabilitation therapy can relieve knee joint pain of PTKS patients,ameliorate the range of motion and function of patients′knee joint,and its effect is superior to conventional rehabilitation therapy alone,which is worthy of clinical application and promotion.
