间歇性呼吸训练对脑卒中偏瘫患者呼气峰流速及运动功能的影响

摘要 目的分析间歇性呼吸训练对脑卒中偏瘫患者呼气峰流速(PEF)及运动功能的影响。方法选择2020年2月至2022年2月我院收治的108例脑卒中偏瘫患者,随机将其分为对照组和研究组,各54例。对照组接受常规康复训练,研究组在此基础上联合间歇性呼吸训练。比较两组的干预效果。结果干预后,研究组的用力肺活量(FVC)、第1秒用力呼气量(FEV_(1))、最大吸气压(MIP)、PEF、最大呼气中段流量(MMEF)高于对照组(P<0.05)。干预后,研究组的24 h正常窦性心搏间期的标准差(SDNN)、24 h内每5 min平均正常RR间期的标准差(SDANN)、24 h相邻R-R间期差值均方根(rMSSD)、相差>50 ms的相邻R-R间期占R-R间期总数百分比(PNN50)高于对照组(P<0.05)。干预后,研究组的0级占比高于对照组,2级占比低于对照组(P<0.05)。干预后,研究组的平衡能力评分高于对照组,步频、步速大于对照组(P<0.05)。结论间歇性呼吸训练有利于改善脑卒中偏瘫患者的呼吸功能、运动功能以及自主神经功能。 Objective To analyze the effect of intermittent breathing training on peak expiratory flow(PEF)and motor function in stroke patients with hemiplegia.Methods A total of 108 stroke patients with hemiplegia admitted in our hospital from February 2020 to February 2022 were selected and randomly divided into control group and study group,with 54 cases in each group.The control group received routine rehabilitation training,and the study group combined with intermittent breathing training on this basis.The intervention effects of the two groups were compared.Results After intervention,the forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),massive inspiratory pressure(MIP),PEF and maximum mid-expiratory flow(MMEF)in the study group were higher than those in the control group(P<0.05).After intervention,the standard deviation of nomal number of intervals(SDNN),standard deviation of the averages of normal number of intervals(SDANN),mean square root of inter-period difference between adjacent R-R at 24 h(rMSSD),percentage of the number of pairs of adjacent R-R intervals differing by more than 50 ms(PNN50)in the study group were higher than those in the control group(P<0.05).After intervention,the proportion of grade 0 in the study group was higher than that in the control group,and the proportion of grade 2 was lower than that in the control group(P<0.05).After intervention,the balance ability score of the study group was higher than that of the control group,and the step frequency and step speed were larger than those of the control group(P<0.05).Conclusion Intermittent breathing training is beneficial to improve the respiratory function,motor function and autonomic nerve function of stroke patients with hemiplegia.
