肝硬化失代偿期患者应用发酵乳辅助治疗对肠道菌群的调节作用

摘要 目的探讨肝硬化失代偿期患者应用发酵乳辅助治疗对肠道菌群的调节作用。方法选取2020年3月至2022年3月上饶市人民医院收治的80例肝硬化失代偿期患者作为研究对象,按照随机数字表法将其分为发酵乳组(n=40)和常规组(n=40)。常规组患者采用常规保肝治疗,发酵乳组在常规组基础上加用发酵乳治疗,均持续治疗8周。分别于治疗前后比较两组患者的胃肠屏障功能、肠道菌群和生活质量。结果治疗后,两组患者的D-乳酸、二胺氧化酶(DAO)和脂多糖(LPS)水平均低于本组治疗前,差异有统计学意义(P<0.05)。发酵乳组患者治疗后的D-乳酸、DAO和LPS水平低于常规组,差异有统计学意义(P<0.05)。治疗后,两组患者的双歧杆菌、乳杆菌、肠杆菌、肠球菌水平高于本组治疗前,差异有统计学意义(P<0.05)。发酵乳组患者治疗后的双歧杆菌和乳杆菌水平高于常规组,肠杆菌和肠球菌含量均低于常规组,差异有统计学意义(P<0.05)。发酵乳组患者治疗后的生活质量高于常规组,差异有统计学意义(P<0.05)。结论应用发酵乳辅助治疗肝硬化失代偿期患者可以较好地改善患者的胃肠屏障功能和肠道菌群平衡,还可以提高患者的生活质量。 Objective To explore the regulation effect of fermented milk adjuvant therapy on intestinal floras in patients with decompensated cirrhosis.Methods A total of 80 patients with decompensated cirrhosis admitted to ShangRao People's Hospital from March 2020 to March 2022 were selected as the research objects,and they were divided into fermented milk group(n=40)and conventional group(n=40)according to random number table method.The patients in the conventional group were treated with conventional liver protection therapy,and the fermented milk group was treated with fermented milk on the basis of the conventional group,and the treatment lasted for 8 weeks.The gastrointestinal barrier function,intestinal flora and quality of life of the two groups were compared before and after treatment.Results After treatment,the levels of D-lactic acid,diamine oxidase(DAO)and lipopolysaccharide(LPS)in two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).The levels of D-lactic acid,DAO and LPS in the fermented milk group after treatment were lower than those in the conventional group,and the differences were statistically significant(P<0.05).After treatment,the levels of bifidobacterium,lactobacillus,enterobacter and enterococcus in two groups were higher than before treatment,and the differences were statistically significant(P<0.05).After treatment,the levels of bifidobacterium and lactobacillus in fermented milk group were higher than those in conventional group,and the contents of enterobacterium and enterococcus were lower than those in conventional group,with statistical significances(P<0.05).The quality of life after treatment in the fermented milk group was higher than that in the conventional group,and the difference was statistically significant(P<0.05).Conclusion The application of fermented milk in the treatment of patients with decompensated liver cirrhosis can improve the gastrointestinal barrier function and intestinal flora balance,and also improve the quality of life of patients.
