无保护会阴接生法在初产妇阴道分娩中的应用效果

Application Effect of Unprotected Perineal Delivery in Primipara Vaginal Delivery

目的:分析无保护会阴接生法在初产妇阴道分娩中的应用效果。方法:选取2020年5月—2022年11月在新沂市中医医院分娩的86例初产妇作为研究对象,根据随机数字表法分为两组,各43例。对照组实施传统会阴保护接生法,观察组实施无保护会阴接生法。比较两组会阴裂伤程度、产程疼痛情况、产妇出血量及新生儿Apgar评分。结果:观察组产妇会阴裂伤程度低于对照组,差异有统计学意义(P=0.001)。观察组产妇产程疼痛评级低于对照组,差异有统计学意义(P=0.020)。观察组产妇出血量少于对照组,差异有统计学意义(P<0.001)。娩出后1、5 min,两组新生儿Apgar评分比较,差异无统计学意义(P>0.05)。结论:无保护会阴接生法可以减轻初产妇会阴裂伤程度,减轻产妇疼痛,减少出血量,且不增加新生儿窒息风险。

Objective:To explore the application effect of unprotected perineal delivery in primipara vaginal delivery.Methods:A total of 86 primiparas who delivered in Xinyi Hospital of Traditional Chinese Medicine from May 2020 to November 2022 were selected as the study subjects.They were divided into two groups according to the method of random number table,with 43 cases in each group.The control group was treated with traditional perineal protection delivery method and the observation group was treated with unprotected perineal delivery method.The degree of perineal laceration,labor pain,maternal blood loss and neonatal Apgar score were compared between the two groups.Results:The degree of perineal laceration in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.001).The labor pain rating level in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.020).The amount of maternal blood loss in the observation group was less than that in the control group,and the difference was statistically significant(P<0.001).At 1 min and 5 min after delivery,there was no significant difference in neonatal Apgar score between the two groups(P>0.05).Conclusion:Unprotected perineal delivery can reduce the degree of perineal laceration,relieve maternal pain,reduce blood loss,and do not increase the risk of neonatal asphyxia.
