低剂量阿替普酶治疗高龄急性缺血性脑卒中患者的短期疗效及安全性分析

Analysis of Short-term Efficacy and Safety of Low-Dose Alteplase in Treatment of Elderly Patients with Acute Ischemic Stroke

目的:探讨低剂量阿替普酶在高龄急性缺血性脑卒中(AIS)患者中的应用效果。方法:选取2017年1月—2021年12月武汉市第一医院收治的86例AIS患者作为研究对象,随机分为观察组(n=50)和对照组(n=36)。观察组接受低剂量(0.6 mg/kg)阿替普酶治疗,对照组接受常规剂量(0.9 mg/kg)阿替普酶治疗。比较两组神经功能、生活自理能力、短期疗效及不良反应发生情况。结果:两组溶栓治疗总有效率比较,差异无统计学意义(P>0.05)。治疗前后,两组神经功能评分、生活自理能力评分比较,差异无统计学意义(P>0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(P=0.046)。结论:低剂量阿替普酶在高龄AIS患者中的应用效果显著,可在保证疗效的同时降低不良反应发生风险。

Objective:To explore the application effect of low-dose alteplase in the elderly patients with acute ischemic stroke(AIS).Methods:A total of 86 AIS patients admitted to Wuhan No.1 Hospital from January 2017 to December 2021 were selected as the study subjects.They were randomly divided into observation group(n=50)and control group(n=36).The observation group was treated with low-dose(0.6 mg/kg)alteplase,and the control group was treated with conventional-dose(0.9 mg/kg)alteplase.The neurological function,self-care ability,short-term efficacy and adverse reactions were compared between the two groups.Results:There was no significant difference in the total effective rate of thrombolytic therapy between the two groups(P>0.05).Before and after treatment,there was no significant difference in neurological function score and self-care ability score between the two groups(P>0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.046).Conclusion:The application of low-dose alteplase in elderly patients with AIS has a significant effect,which can ensure the efficacy and reduce the risk of adverse reactions.
