小柴胡汤合瓜蒌红花散加减联合常规西药治疗带状疱疹后遗神经痛的效果观察

Effect Observation of Xiaochaihu Decoction and Gualou Honghua Powder Modification Combined with Western Medicine in Treatment of Postherpetic Neuralgia

目的:分析小柴胡汤合瓜蒌红花散加减联合常规西药治疗带状疱疹后遗神经痛的临床效果。方法:选取2021年12月—2022年10月北京市东城区天坛社区卫生服务中心收治的74例带状疱疹后遗神经痛患者作为研究对象,按照随机数字表法分为对照组和观察组,各37例。对照组采用常规西药治疗,观察组在对照组基础上应用小柴胡汤合瓜蒌红花散加减治疗。比较两组症状积分、疼痛程度、睡眠质量、炎性因子水平。结果:观察组疱疹消退后色素沉着、疲乏、刺痛、夜寐不安、舌黯紫苔白、脉细积分均低于对照组,差异有统计学意义(P<0.05)。治疗后10、20、30 d,观察组疼痛、匹兹堡睡眠质量指数评分均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组白细胞介素-2、白细胞介素-6、肿瘤坏死因子-α、干扰素-γ水平均低于对照组,差异有统计学意义(P<0.05)。结论:小柴胡汤合瓜蒌红花散加减联合常规西药治疗带状疱疹后遗神经痛的效果显著,可有效改善患者临床症状,缓解疼痛程度,提升睡眠质量,减轻炎性反应。

Objective:To analyze the clinical effect of Xiaochaihu Decoction and Gualou Honghua Powder modification combined with western medicine in the treatment of postherpetic neuralgia.Methods:A total of 74 patients with postherpetic neuralgia admitted to Beijing Dongcheng District Tiantan Community Health Service Center from December 2021 to October 2022 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 37 cases in each group.The control group was treated with conventional western medicine and the observation group was treated with Xiaochaihu Decoction and Gualou Honghua Powder modification on the basis of control group.Symptom scores,pain levels,sleep quality,and inflammatory factor levels were compared between the two groups.Results:After herpes regression,the pigmentation fatigue,stabbing pain,restless sleep,dark purple tongue and white tongue coating,thready pulse scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).At 10 d,20 d and 30 d after treatment,the pain and Pittsburgh sleep quality index scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of interleukin-2,interleukin-6,tumor necrosis factor-αand interferon-γin the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Xiaochaihu Decoction and Gualou Honghua Powder modification combined with western medicine has a significant effect in the treatment of postherpetic neuralgia,which can effectively improve the clinical symptoms,relieve pain level,improve sleep quality and reduce inflammatory reaction.
