连续护理模式在老年慢性病患者社区护理中的应用效果分析

Analysis on Application Effect of Continuous Nursing Mode in Community Nursing of Elderly Patients with Chronic Diseases

目的:分析连续护理模式在老年慢性病患者社区护理中的应用效果。方法:选取2021年1月—2022年2月威海市东方社区卫生服务中心收治的110例老年慢性病患者作为研究对象,根据随机数字表法分为对照组和观察组,各55例。对照组采用常规护理,观察组采用连续护理。比较两组负性情绪、护理满意度及自我护理能力。结果:干预前,两组焦虑自评量表(SAS)和抑郁自评量表(SDS)评分比较,差异无统计学意义(P>0.05);干预后,两组SAS、SDS评分低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组护理总满意度高于对照组,差异有统计学意义(P<0.05)。干预前,两组自我护理意识、自我护理技能、健康知识水平评分比较,差异无统计学意义(P>0.05);干预后,两组自我护理意识、自我护理技能、健康知识水平评分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:连续护理模式在老年慢性病患者社区护理中的应用效果显著,可有效缓解患者负性情绪,提高护理满意度及自我护理能力。

Objective:To analyze the application effect of continuous nursing mode in community nursing of elderly patients with chronic diseases.Methods:A total of 110 elderly patients with chronic diseases admitted to Dongfang Community Health Service Center of Weihai from January 2021 to February 2022 were selected as the study objects and divided into control group and observation group according to random number table method,with 55 cases in each group.The control group received routine nursing and the observation group received continuous nursing.The negative emotions,nursing satisfaction and self-care ability of the two groups were compared.Results:Before intervention,there was no significant difference in Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)scores between the two groups(P>0.05).After intervention,the SAS and SDS scores in the two groups were lower than those before intervention,the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The total satisfaction of nursing in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Before intervention,there was no significant difference in self-care awareness,self-care skills and health knowledge scores between the two groups(P>0.05).After intervention,the scores of self-care awareness,self-care skills and health knowledge in the two groups were higher than those before intervention,the observation group was higher than the control group,and the differences were statistically significant(P<0.05).Conclusion:The application of continuous nursing mode in community nursing of elderly patients with chronic diseases has significant effects,which can effectively relieve the negative emotions of patients,and improve nursing satisfaction and self-care ability.
