止痛生肌散联合手术治疗下肢静脉曲张性溃疡的效果及其对下肢血流灌注的影响

Effect of zhitongshengji powder combined with surgery in the treatment of varicose ulcers of lower extremity and its impact on blood perfusion of lower extremity

目的探讨止痛生肌散联合手术治疗下肢静脉曲张性溃疡的效果及其对下肢血流灌注的影响。方法收集2019年1月至2020年12月于石家庄市中医院就诊的115例下肢静脉曲张性溃疡患者的临床资料,按照治疗方法的不同将其分为对照组(n=52,采用手术治疗)和治疗组(n=63,在对照组基础上联合止痛生肌散治疗)。比较两组患者的临床疗效及治疗前后溃疡面积、中医证候积分、下肢血流灌注情况。结果治疗后,两组患者溃疡面积均明显小于本组治疗前,且治疗组患者溃疡面积明显小于对照组患者,差异均有统计学意义(P﹤0.01)。治疗后,两组患者疼痛、溃疡表面色泽、创面渗液量积分均低于本组治疗前,且治疗组患者上述评分均低于对照组患者,差异均有统计学意义(P﹤0.05)。治疗后,两组患者血流速度、静脉血流峰速度均明显高于本组治疗前,且治疗组患者上述指标均明显高于对照组患者,差异均有统计学意义(P﹤0.01)。治疗组患者治疗总有效率为100%(63/63),高于对照组患者的90.38%(47/52),差异有统计学意义(P﹤0.05)。结论止痛生肌散联合手术治疗可有效改善下肢静脉曲张性溃疡患者的临床症状及下肢血流灌注情况,促进创面愈合,临床疗效显著。

Objective To investigate the effect of zhitongshengji powder combined with surgery in the treatment of varicose ulcers of lower extremity and its impact on blood perfusion of lower extremity.Method The clinical data of 115 patients with varicose ulcers of lower extremity who visited Shijiazhuang Traditional Chinese Medicine Hospital from January 2019 to December 2020 were collected.According to different treatment methods,they were divided into control group(n=52,treated with surgery)and treatment group(n=63,treated with zhitongshengji powder on the basis of the control group).The clinical efficacy,ulcer area,TCM syndrome scores,and lower extremity blood perfusion before and after treatment were compared between the two groups of patients.Result After treatment,the ulcer area of the two groups of patients was significantly smaller than before the treatment,and the ulcer area of the treatment group was significantly smaller than those of the control group,the differences were statistically significant(P<0.01).After treatment,the pain,ulcer surface color,and wound exudate volume scores of the two groups of patients were all lower than before the treatment,and the above scores of the patients in the treatment group were lower than those of the patients in the control group,the differences were statistically significant(P<0.05).After treatment,the blood flow velocity and venous blood flow peak velocity of the two groups of patients were significantly higher than before the treatment,and the above indicators of the patients in the treatment group were significantly higher than those of the patients in the control group,the differences were statistically significant(P<0.01).The total effective rate in the treatment group was 100%(63/63),higher than 90.38%(47/52)in the control group,the difference was statistically significant(P<0.05).Conclusion The combination of zhitongshengji powder and surgical treatment could effectively improve the clinical symptoms of patients with varicose vein ulcer and blood perfusion in the lower extremity,promote wound healing,it has remarkable clinical efficacy.
