内镜模拟器及机械臂在口腔颌面外科住院医师临床前技能培训中的应用

Application of Endoscopy Simulator and Surgical Robot Manipulator in the Preclinical Skill Training for Residents of Oral and Maxillofacial Surgery

摘要 目的:探索内镜模拟器结合机械臂在口腔颌面外科住院医师临床前技能培训中的应用价值,以寻求更为有效的口腔颌面外科手术机器人机械臂临床前培训模式。方法:纳入四川大学华西口腔医学院口腔颌面外科住院医师规范化培训学员60人,随机分为两组,一组为内镜模拟器结合机械臂培训组(实验组),一组为单纯机械臂培训组(对照组),培训后对两组学员进行机械臂操作考核,根据考核成绩绘制学习曲线;同时对学员进行满意度调查,评价该培训模式对住院医师临床前技能提升的效果。结果:两组学员的首次完成机械臂考核用时差异无统计学意义(t=-1.437,P> 0.05);机械臂训练之后实验组学员完成时间明显少于对照组(P <0.001),学习曲线缩短;实验组学员对其接受培训的各方面满意度均显著高于对照组,差异具有统计学意义(P <0.05)。结论:内镜模拟器结合机械臂培训模式能够有效提高口腔颌面外科住院医师临床前技能水平,教学满意度良好。该教学体系的推广有利于推动内镜技术与手术机器人在口腔颌面外科的应用和发展。 Objective:To assess the values of endoscopy simulator and surgical robot manipulator in the application of preclinical skill training for residents of oral and maxillofacial surgery,and explore a more efficient training approach in improving the capability to operate surgical robots.Methods:A randomized study was carried out among 60 residents from the department of oral and maxillofacial surgery in West China School of Stomatology,Sichuan University.Thirty residents were assigned to the endoscopy simulator combined with surgical robot manipulator training group(the experiment group),and another 30 subjects underwent the manipulator only training(the control group).The operation test was conducted after training to evaluate the manipulating skills of the trainees in two groups,and their performance curves were made based on their scores.A training satisfaction survey was carried out among all the trainees.Results:There was no significant difference between these two groups in terms of the time in completing the test before training(t =-1.437,P> 0.05),the time in the experiment group,however,was significantly shorter than that in the control group after training(P< 0.001),as is shown in the performance curves.The trainees in the experiment group were obviously more satisfied with the training mode they experienced than those in the control group(P< 0.05).Conclusion:The training pattern combining endoscopy simulator and surgical robot manipulator could efficiently enhance the preclinical skills of residents of oral and maxillofacial surgery,and this teaching approach possesses favourable satisfaction among trainees,which deserves to be promoted.
