舒芬太尼与芬太尼复合丙泊酚在宫腔镜手术中的应用效果比较

Comparison of the application effect of Sufentanil and Fentanyl combined with Propofol in hysteroscopic surgery

目的比较舒芬太尼与芬太尼复合丙泊酚在宫腔镜手术中的应用效果。方法选取2020年6月至2023年6月在揭阳市中医院接受宫腔镜手术的60例患者作为研究对象,使用随机数字表法分为研究组和对照组,各30例。两组均进行静脉全身麻醉,在麻醉诱导前,研究组静脉注射舒芬太尼0.2μg/kg,对照组静脉注射芬太尼1μg/kg。两组麻醉诱导静脉注射丙泊酚2 mg/kg,麻醉维持静脉泵注丙泊酚4 mg/(kg·h),出现体动反应时静脉注射丙泊酚0.5 mg/kg。记录两组丙泊酚使用总量、术后苏醒时间、苏醒时视觉模拟评分法(VAS)评分及围手术期恶心呕吐、眩晕发生率。结果研究组丙泊酚使用总量、术后苏醒时间、苏醒时VAS评分较对照组降低(P<0.05);两组恶心呕吐、眩晕发生率比较,差异无统计学意义(P>0.05)。结论与芬太尼复合丙泊酚比较,舒芬太尼复合丙泊酚用于宫腔镜手术镇痛效果较好,可减少丙泊酚使用总量,缩短术后苏醒时间且不增加围手术期恶心呕吐、眩晕发生率。

Objective To compare the application effect of Sufentanil and Fentanyl combined with Propofol in hysteroscopic surgery.Methods A total of 60 patients who underwent hysteroscopic surgery in Jieyang City Chinese Medicine Hospital from June 2020 to June 2023 were selected as the study objects.All the patients were divided into the study group and the control group using the random number table method,with 30 cases in each group.Both groups received intravenous general anesthesia.Before induction of anesthesia,Sufentanil 0.2μg/kg was injected intravenously in the study group,and 1μg/kg Fentanyl was injected intravenously in the control group.The two groups received Propofol 2 mg/kg intravenously for induction of anesthesia,Propofol 4 mg/(kg·h)intravenously for maintenance of anesthesia,and 0.5 mg/kg Propofol intravenously for patients with body movement reactions.The total amount of Propofol used,postoperative recovery time and visual analogue scale(VAS)score at recovery and the incidence of perioperative nausea,vomiting and vertigo of the two groups were recorded.Results The total amount of Propofol used,postoperative recovery time,and VAS score at recovery of the study group were lower than those of the control group(P<0.05).There was no significant difference in the incidence of nausea,vomiting and vertigo between the two groups(P>0.05).Conclusion Compared with Fentanyl combined with Propofol,Sufentanil combined with Propofol has a better analgesic effect in hysteroscopic surgery,which can reduce the total amount of Propofol used,shorten postoperative recovery time and does not increase the incidence of perioperative nausea vomiting and dizziness.
