不同剂量盐酸右美托咪定在腹腔镜子宫切除手术麻醉中的应用效果对比

Comparison of application effect of different doses of Dexmede-tomidine Hydrochloride in anesthesia for laparoscopic hysterectomy

目的对比不同剂量盐酸右美托咪定在腹腔镜子宫切除手术麻醉中的应用效果。方法选取2021年3月至2023年6月宁夏回族自治区第五人民医院的153例行腹腔镜子宫切除手术治疗的患者作为研究对象,通过随机数字表法分为低剂量组(76例)、高剂量组(77例)。低剂量组泵注0.2μg/kg盐酸右美托咪定进行麻醉,高剂量组泵注0.4μg/kg盐酸右美托咪定进行麻醉。比较两组术后麻醉恢复时间、不同时间点血流动力学及不良反应发生情况。结果高剂量组的自主呼吸、苏醒、拔管时间长于低剂量组(P<0.05)。在泵入盐酸右美托咪定后10 min、气管拔管时、拔管后10 min,高剂量组的心率、平均动脉压低于低剂量组(P<0.05)。高剂量组不良反应发生率低于低剂量组(P<0.05)。结论在腹腔镜子宫切除手术麻醉中泵注0.4μg/kg盐酸右美托咪定可有效促进术后麻醉恢复,稳定血流动力学,降低不良反应发生率。

Objective To compare the application effect of different doses of Dexmedetomidine Hydrochloride in anesthesia for laparoscopic hysterectomy.Methods A total of 153 patients undergoing laparoscopic hysterectomy in the Fifth People’s Hospital of Ningxia from March 2021 to June 2023 were selected as the study objects.All the patients were divided into the low-dose group(76 cases)and the high-dose group(77 cases)using the random number table method.The low-dose group was anesthetized by pumping 0.2μg/kg Dexmedetomidine Hydrochloride,and the high-dose group was anesthetized by pumping 0.4μg/kg Dexmedetomidine Hydrochloride.The postoperative anesthesia recovery time,hemodynamics at different time points and the incidence of adverse reactions were compared between the two groups.Results Autonomous respiration,awakening and extubation time of the high-dose group was longer than that of the low-dose group(P<0.05).At 10 min after pumping Dexmedetomidine Hydrochloride,at the time of tracheal extubation,and at 10 min after extubation,the heart rate and mean arterial pressure of the high-dose group were lower than those of the low-dose group(P<0.05).The incidence of adverse reactions of the high-dose group was lower than that of the low-dose group(P<0.05).Conclusion Pumping 0.4μg/kg Dexmedetomidine Hydrochloride during anesthesia for laparoscopic hysterectomy can effectively promote postoperative anesthesia recovery,stabilize hemodynamics,and reduce the incidence of adverse reactions.
