垂直高强线NICE结缝合联合Krackow缝合治疗髌骨下极骨折的临床应用

Clinical Application of Vertical High Strength Thread NICE Suturing Combined with Krackow Suturing in the Treatment of Patellar Inferior Pole Fractures

目的:比较使用垂直高强线NICE结缝合联合Krackow缝合与使用垂直钢丝缝合联合Krackow缝合治疗髌骨下极骨折的疗效。方法:回顾性分析髌骨下极骨折患者35例,根据内固定方式不同分组,A组19例采用垂直高强线NICE结缝合联合Krackow缝合治疗。B组16例采用垂直钢丝固定联合Krackow缝合。结果:所有患者影像学上均骨性愈合。35例患者取得随访,平均随访时间(10.09±3.05)月。术后2周、1月、3月随访Bostman评分两组比较无显著性差异(P>0.05);术后6月随访Bostman评分A组优于B组(P=0.027)。结论:垂直高强线NICE结缝合联合Krackow缝合与使用垂直钢丝固定联合Krackow缝合两种方法治疗髌骨下极骨折均能取得良好的临床疗效,垂直高强线NICE结缝合联合Krackow缝合具有不用取出内固定,避免金属内植物对皮肤激惹等优点,是治疗髌骨下极骨折的可选择方案。

Objective:To compare the effect of NICE junction suture with Krackow suture and vertical wire suture with Krackow suture in the treatment of lower pole fracture of patella.Methods:35 patients with lower pole fracture of patella were retrospectively analyzed.According to different internal fixation methods,19 patients in group A were treated with vertical high-strength line NICE suture combined with Krackow suture.16 patients in group B received vertical wire fixation combined with Krackow suture.Result:All patients showed bone union on imaging.Thirty-five patients were followed up with an average follow-up time of 10.09±3.05 months.There was no significant difference in Bostman score between the two groups at 2 weeks,1 month and 3 months after surgery(>0.05).Bostman score in group A was better than that in group B(=0.027).Conclusion:Both vertical high-strength suture with NICE suture combined with Krackow suture and vertical wire fixation combined with Krackow suture can achieve good clinical efficacy in the treatment of lower pole fracture of patella.Vertical high-strength suture with NICE suture combined with Krackow suture has the advantages of not taking out internal fixation and avoiding the irritation of metal plants on the skin.It is an optional treatment for fracture of the lower pole of patella.
