输尿管软镜碎石与经皮肾镜碎石对3 cm以下肾结石疗效和炎性反应的影响

Effect of Flexible Ureteroscopic Lithotripsy and Percutaneous Nephrolithotripsy on the Efficacy and Inflammatory Response of Renal Calculi Less Than 3 cm in Size

目的探讨对3 cm以下肾结石予输尿管软镜与经皮肾镜碎石治疗的效果及对炎症反应的影响。方法选择2020年4月—2023年2月金湖县人民医院收治的直径3 cm以下肾结石患者86例,随机数表法分为经皮肾镜碎石对照组(n=43)与输尿管软镜碎石治疗观察组(n=43),比较两组患者手术相关指标、碎石成功率及并发症率、炎性反应水平。结果观察组各项手术相关指标均优于对照组,差异有统计学意义(P<0.05)。两组患者结石清除率比较,差异无统计学意义(P>0.05)。观察组并发症率低于对照组(2.33%vs 18.60%),差异有统计学意义(χ^(2)=4.468,P<0.05)。术后观察组炎性反应水平低于对照组,差异有统计学意义(P<0.05)。结论对3 cm以下肾结石予输尿管软镜与经皮肾镜治疗疗效相当,输尿管软镜操作简单、损伤小、疼痛轻,并发症少,能减炎性反应。

Objective To investigate the effect of flexible ureteroscopic lithotripsy and percutaneous nephrolithotripsy on the treatment of renal calculi less than 3 cm in size and the effect on inflammatory reaction.Methods A total of 86 patients with kidney stones less than 3 cm in diameter admitted to Jinhu County People's Hospital from April 2020 to February 2023 were randomly divided into percutaneous nephrolithotripsis control group(n=43)and ureteral soft litho-tripsis observation group(n=43)by random number table method.The surgical indicators,lithotripsis success rate,complication rate and inflammatory response level of the two groups were compared.Results All surgical related in-dexes in the observation group were better than those in the control group,and the difference was statistically signifi-cant(P<0.05).There was no statistically significant difference in stone clearance between the two groups(P>0.05).The complication rate of the observation group was lower than that of the control group(2.33%vs18.60%),and the dif-ference was statistically significant(χ^(2)=4.468,P<0.05).The inflammatory response level of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The efficacy of flexible ureteroscopic and percutaneous nephrolithotomy for renal calculi less than 3 cm is comparable,and ure-teroscopy is simple,less damaging,less painful,with fewer complications,and reduces inflammatory reactions.
