腹腔镜手术联合醋酸亮丙瑞林治疗子宫内膜异位症的效果

Efficacy of Laparoscopic Surgery Combined with Leuprolide Acetate in the Treatment of Endometriosis

目的:分析腹腔镜手术联合醋酸亮丙瑞林治疗子宫内膜异位症的效果。方法:回顾性分析2020年1月—2021年12月厦门大学附属第一医院收治的140例子宫内膜异位症患者的临床资料。根据治疗方案的不同将其分为参照组和验证组,各70例。两组均给予腹腔镜手术。参照组术后给予孕三烯酮胶囊,验证组术后给予注射用醋酸亮丙瑞林缓释微球。比较两组临床疗效,治疗前后性激素、卵巢相关指标,复发率及不良反应。结果:验证组总有效率(94.29%)高于参照组(80.00%),差异有统计学意义(P<0.05)。治疗后,验证组雌二醇(E_(2))、促黄体生成素(LH)、卵泡刺激素(FSH)水平均明显低于参照组,差异有统计学意义(P<0.05)。治疗后,验证组卵巢体积小于参照组,窦卵泡计数(AFC)多于参照组,差异有统计学意义(P<0.05)。验证组复发率为7.58%(5/66),低于参照组的21.43%(12/56),差异有统计学意义(χ^(2)=5.039,P=0.025)。验证组不良反应发生率(5.71%)低于参照组(17.14%),差异有统计学意义(P<0.05)。结论:腹腔镜手术联合醋酸亮丙瑞林治疗子宫内膜异位症效果确切,可降低患者E_(2)、LH、FSH水平,改善患者卵巢功能,降低复发率,且安全性高。

Objective:To analyze the efficacy of laparoscopic surgery combined with Leuprolide Acetate in the treatment of endometriosis.Method:The clinical data of 140 patients with endometriosis admitted to the First Affiliated Hospital of Xiamen University from January 2020 to December 2021 were retrospectively analyzed.They were divided into reference group and verification group according to different treatment plans,70 cases in each group.Both groups were given laparoscopic surgery.The reference group was given Gestrinone Capsules after surgery,and the verification group was given Leuprorelin Acetate Microspheres Sustained Release For Injection after surgery.The clinical efficacy,sex hormone,ovary related indexes before and after treatment,recurrence rate and adverse reactions were compared between the two groups.Result:The total effective rate of the verification group(94.29%)was higher than that of the reference group(80.00%),and the difference was statistically significant(P<0.05).After treatment,the levels of estradiol(E2),luteinizing hormone(LH)and follicle stimulating hormone(FSH)in the verification group were significantly lower than those in the reference group,and the differences were statistically significant(P<0.05).After treatment,the ovarian volume of the verification group was smaller than that of the reference group,and the antral follicle count(AFC)was more than that of the reference group,and the differences were statistically significant(P<0.05).The recurrence rate in the verification group was 7.58%(5/66),which was lower than 21.43%(12/56)in the reference group,and the difference was statistically significant(X^(2)=5.039,P=0.025).The incidence of adverse reactions in verification group(5.71%)was lower than that in reference group(17.14%),and the difference was statistically significant(P<0.05).Conclusion:Laparoscopic surgery combined with Leuprolide Acetate has a definite effect in the treatment of endometriosis,which can reduce the levels of E2,LH and FSH in patients,improve the ovarian function of patients,and reduce the recurrence rate,with high safety.
