雷公藤多苷片联合阿魏酸哌嗪片治疗IgA肾病的临床效果

Clinical Effect of Tripterygium Wilfordii Polyglycoside Tablets Combined with Piperazine Ferulate Tablets in the Treatment of IgA Nephropathy

目的:观察雷公藤多苷片联合阿魏酸哌嗪片治疗免疫球蛋白A(IgA)肾病的临床效果。方法:选取2021年11月—2022年11月成都中医药大学附属医院收治的92例Ig A肾病患者。根据随机数表法将其分为对照组和研究组,各46例。对照组给予雷公藤多苷片,研究组在对照组基础上给予阿魏酸哌嗪片。比较两组治疗前及治疗12周后24 h尿蛋白定量、炎症因子、血小板聚集率、肾功能指标及临床疗效。结果:治疗12周后,两组24 h尿蛋白定量均低于治疗前,研究组24 h尿蛋白定量低于对照组,差异有统计学意义(P<0.05)。治疗12周后,两组肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平均低于治疗前,研究组TNF-α及IL-6水平均低于对照组,差异有统计学意义(P<0.05)。治疗12周后,两组血小板聚集率低于治疗前,研究组血小板聚集率低于对照组,差异有统计学意义(P<0.05)。治疗12周后,两组血肌酐(Scr)低于治疗前,估算肾小球滤过率(eGFR)高于治疗前,研究组Scr低于对照组,eGFR高于对照组,差异有统计学意义(P<0.05)。研究组总有效率高于对照组,差异有统计学意义(P<0.05)。结论:雷公藤多苷片联合阿魏酸哌嗪片治疗Ig A肾病效果显著,能够有效减少24 h尿蛋白定量,降低炎症因子水平,改善肾功能及血小板聚集率,提高治疗效果。

Objective:To observe the clinical effect of Tripterygium Wilfordii Polyglycoside Tablets combined with Piperazine Ferulate Tablets in the treatment of immunoglobulin A(IgA)nephropathy.Method:Ninety-two patients with IgA nephropathy admitted to the Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from November 2021 to November 2022 were selected.According to the method of random number table,they were divided into control group and study group,46 cases in each group.The control group was given Tripterygium Wilfordii Polyglycoside Tablets,and the study group was given Piperazine Ferulate Tablets based on the control group.24 h urinary protein quantity,inflammatory factors,platelet aggregation rate,renal function indexes before and after 12 weeks of treatment and clinical efficacy of the two groups were compared.Result:After 12 weeks of treatment,24 h urinary protein quantity in both groups were lower than those before treatment,and 24 h urinary protein quantity in study group was lower than that in control group,the differences were statistically significant(P<0.05).After 12 weeks of treatment,the levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in both groups were lower than those before treatment,and the levels of TNF-αand IL-6 in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After 12 weeks of treatment,the platelet aggregation rate in the two groups were lower than those before treatment,and the platelet aggregation rate in the study group was lower than that in the control group,the differences were statistically significant(P<0.05).After 12 weeks of treatment,serum creatinine(Scr)and estimated glomerular filtration rate(eGFR)in both groups were lower than those before treatment,Scr in study group was lower than that in control group,eGFR was higher than that in control group,the differences were statistically significant(P<0.05).The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Tripterygium Wilfordii Polyglycoside Tablets combined with Piperazine Ferulate Tablets has a significant effect in the treatment of IgA nephropathy,which can effectively reduce 24 h urinary protein,reduce the inflammatory factor level,improve the renal Chinese and Foreign Medical Research Vol.21,No.30 October,2023 function and platelet aggregation rate,and improve the therapeutic effect.
