接纳与承诺疗法联合同伴支持对艾滋病患者希望水平、应对方式及病耻感的影响

Effects of Acceptance and Commitment Therapy Combined with Peer Support on Hope Levels,Coping Styles and Stigma of AIDS Patients

目的:探究接纳与承诺疗法联合同伴支持对艾滋病患者希望水平、应对方式及病耻感的影响。方法:选择2021年1月—2022年2月厦门大学附属第一医院收治的106例艾滋病患者为研究对象,利用随机数表法将患者随机分为对照组和研究组,各53例。给予对照组患者常规护理干预,给予研究组患者接纳与承诺疗法联合同伴支持干预。比较两组希望水平、应对方式、病耻感、心理弹性及护理满意度。结果:干预后,两组Herth希望量表(HHI)评分、社会影响量表(SIS)评分均优于干预前,研究组HHI评分高于对照组,SIS评分低于对照组,差异有统计学意义(P<0.05)。干预后,两组面对、屈服、回避评分均优于干预前,研究组面对评分高于对照组,屈服、回避评分均低于对照组,差异有统计学意义(P<0.05)。干预后,两组心理弹性评分均高于干预前,研究组高于对照组,差异有统计学意义(P<0.05)。研究组患者的护理总满意度高于对照组,差异有统计学意义(P<0.05)。结论:接纳与承诺疗法联合同伴支持对艾滋病患者具有积极影响,可以提高患者的心理弹性,增加患者的希望水平,同时还能减轻患者的病耻感,改变患者的应对方式,使患者的护理满意度得到提升。

Objective:To explore the effects of acceptance and commitment therapy combined with peer support on hope coping style and stigma of AIDS patients.Method:A total of 106 AIDS patients admitted to the First Affiliated Hospital of University from January 2021 to February 2022 were selected as the study objects.The patients were randomly divided into control group and study group by random number table method,with 53 cases in each group.The control group was given routine nursing intervention,and the study group was given acceptance and commitment therapy combined with peer support intervention.Hope level,coping style,stigma,psychological resilience and nursing satisfaction were compared between the two groups.Result:After intervention,Herth hope index(HHI)score and social impact scale(SIS)score of the two groups were better than those before intervention,HHI score of the study group was higher than that of the control group,and SIS score was lower than that of the control group,the differences were statistically significant(P<0.05).After intervention,the scores of face,yield and avoidance in both groups were better than those before intervention,the score of face in the study group was higher than that in the control group,and the scores of yield and avoidance were lower than those in the control group,the differences were statistically significant(P<0.05).After intervention,the psychological resilience scores of both groups were higher than those before intervention,and the study group was higher than the control group,the differences were statistically significant(P<0.05).The total satisfaction of nursing in the study group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Acceptance and commitment therapy combined with peer support has a positive impact on AIDS patients,which can improve patients'psychological resilience,increase patients'hope level,reduce patients'stigma,change patients'coping styles,and improve patients'nursing satisfaction.
