尿液NGAL、血Cys C及Scr在高血压急性肾损伤中的变化及与疾病分期的关系

Changes of Urine NGAL,Blood Cys C and Scr in Hypertensive Acute Kidney Injury and Their Relationship with Disease Stage

目的:探究尿液中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、血胱抑素C(Cys C)及血肌酐(Scr)在高血压急性肾损伤中的变化及与疾病分期的关系。方法:选取2021年5月—2023年1月厦门大学附属第一医院收治的40例高血压急性肾损伤1期患者为A组,40例高血压急性肾损伤2期患者为B组,40例高血压急性肾损伤3期患者为C组,40例无肾损伤的高血压患者为D组。比较四组尿液NGAL、血Cys C及Scr检测水平与阳性率,采用Spearman等级相关分析高血压急性肾损伤患者尿液NGAL、血Cys C及Scr与疾病分期的关系。结果:A组、B组及C组的尿液NGAL、血Cys C及Scr检测水平与阳性率均显著高于D组,B组及C组的尿液NGAL、血Cys C及Scr检测水平与阳性率均显著高于A组,差异有统计学意义(P<0.05)。C组的尿液NGAL、血Cys C及Scr检测水平高于B组,差异有统计学意义(P<0.05)。Spearman等级相关分析显示,高血压急性肾损伤患者尿液NGAL、血Cys C及Scr与疾病分期呈正相关(P<0.05)。结论:高血压急性肾损伤患者的尿液NGAL、血Cys C及Scr均相对更高,且与疾病分期呈正相关,在高血压急性肾损伤患者中的检测价值较高。

Objective:To explore the changes of urine neutrophil gelatinase-associated lipocalin(NGAL),blood Cystatin C(Cys C)and Serum creatinine(Scr)in hypertensive acute kidney injury and their relationship with disease stage.Method:Forty patients with stage 1 hypertensive acute kidney injury in the First Affiliated Hospital of Xiamen University from May 2021 to January 2023 were selected as group A,40 patients with stage 2 hypertensive acute kidney injury as group B,and 40 patients with stage 3 hypertensive acute kidney injury as group C,40 hypertensive patients without renal injury were in group D.Urine NGAL,blood Cys C and Scr detection levels and positive rates of the four groups were compared.Spearman correlation was used to analyze the relationship between urine NGAL,blood Cys C and Scr and disease stage in patients with hypertensive acute kidney injury.Result:The urine NGAL,blood Cys C and Scr detection levels and positive rate of group A,group B and group C were significantly higher than those of group D,the urine NGAL,blood Cys C and Scr detection levels and positive rate of group B and group C were significantly higher than those of group A,and the differences were statistically significant(P<0.05).The levels of urine NGAL,blood Cys C and Scr in group C were higher than those in group B,and the differences were statistically significant(P<0.05).Spearman grade correlation analysis showed that urine NGAL,blood Cys C and Scr were positively correlated with disease stage in patients with hypertensive acute kidney injury(P<0.05).Conclusion:Urine NGAL,blood Cys C and Scr are relatively higher in patients with hypertensive acute kidney injury,and are positively correlated with disease stage,which is of high detection value in patients with hypertensive acute kidney injury.
