早期标准化肠内营养支持在内镜黏膜下剥离术后胃癌患者中的应用效果

Application Effect of Early Standardized Enteral Nutrition Support in Patients with Gastric Cancer after Endoscopic Submucosal Decollement

摘要 目的:探讨早期标准化肠内营养支持在内镜黏膜下剥离术后胃癌患者中的应用效果。方法:选取2020年1月—2022年12月莆田市第一医院收治的68例内镜黏膜下剥离术后胃癌患者作为研究对象。根据随机数表法将其分为对照组和观察组,各34例。对照组给予常规营养支持,观察组给予早期标准化肠内营养支持。比较两组肠内营养不耐受情况、并发症、肠道恢复情况,干预前、干预后2周功能状况、营养指标。结果:观察组腹泻、腹痛、腹胀、出血、管腔狭窄发生率均低于对照组,差异有统计学意义(P<0.05)。观察组首次排气时间、首次排便时间均明显早于对照组,干预后2周Karnofsky功能状态量表(KPS)评分明显高于对照组,差异有统计学意义(P<0.05)。干预后2周,观察组总蛋白、血红蛋白、免疫球蛋白G水平均明显高于对照组,差异有统计学意义(P<0.05)。结论:内镜黏膜下剥离术后胃癌患者给予早期标准化肠内营养支持能有效调节患者营养状况,改善机体功能状态,还能提高营养支持耐受性、降低手术并发症及促进肠道功能恢复。 Objective:To investigate the application effect of early standardized enteral nutrition support in patients with gastric cancer after endoscopic submucosal decollement.Method:Sixty-eight patients with gastric cancer after endoscopic submucosal decollement treated in Putian First Hospital from January 2020 to December 2022 were selected as the study objects.They were divided into control group and observation group according to random number table method,34 cases in each group.The control group received conventional nutrition support,and the observation group received early standardized enteral nutrition support.Enteral nutritional intolerance condition,complications,intestinal recovery condition,functional status and nutritional indexes before intervention and 2 weeks after intervention were compared between the two groups.Result:The incidences of diarrhea,abdominal pain and distension,bleeding and lumen stenosis in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The first exhaust time and first defecation time in the observation group were significantly earlier than those in the control group,and the Karnofsky functional status scale(KPS)score at 2 weeks after intervention was significantly higher than that in the control group,the differences were statistically significant(P<0.05).At 2 weeks after intervention,the levels of total protein,hemoglobin and immunoglobulin G in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Early standardized enteral nutrition support for gastric cancer patients after endoscopic submucosal decollement can effectively regulate the nutritional status of patients,improve the body function status,improve the tolerance of nutritional support,reduce surgical complications and promote the recovery of intestinal function.
