有氧运动训练配合呼吸康复训练对肺叶切除术后患者肺功能、运动耐力及生活质量的影响

摘要 目的:探讨有氧运动训练配合呼吸康复训练对肺叶切除术后患者肺功能、运动耐力及生活质量的影响。方法:选择2021年11月—2023年4月于罗定市人民医院行肺叶切除术的84例患者,以随机数表法分为两组,各42例。对照组接受常规护理及常规训练,观察组在对照组基础上采取有氧运动训练配合呼吸康复训练,两组均干预4周。比较两组干预前后的肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV_1)]、运动耐力[6分钟步行试验(6MWT)]、疲劳耐受程度[疲劳量表-14(FS-14)]及生活质量[健康调查简表(SF-36)]。结果:干预前,两组肺功能指标比较,差异无统计学意义(P>0.05);干预后,两组FVC、FEV_1均高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。干预前,两组运动耐力及疲劳耐受程度比较,差异无统计学意义(P>0.05);干预后,两组6MWT距离长于干预前,FS-14评分低于干预前,且观察组6MWT距离长于对照组,FS-14评分低于对照组,差异有统计学意义(P<0.05)。干预前,两组生活质量比较,差异无统计学意义(P>0.05);干预后,两组SF-36总分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:肺叶切除术后患者采取有氧运动训练配合呼吸康复训练,可有效提升肺功能,改善运动耐力及疲劳耐受度,提升生活质量。 Objective:To explore the effects of aerobic exercise training combined with respiratory rehabilitation training on lung function,exercise endurance and quality of life in patients after pulmonary lobectomy.Method:A total of 84 patients who underwent pulmonary lobectomy in Luoding People's Hospital from November 2021 to April 2023 were selected and divided into two groups by random number table method,with 42 patients in each group.The control group received routine nursing and routine training,and the observation group received aerobic exercise training combined with respiratory rehabilitation training on the basis of the control group.Both groups were intervened for 4 weeks.The lung function indexes[forced vital capacity(FVC),forced expiratory volume in one second(FEV1)],exercise endurance[6 minutes walking test(6MWT)],fatigue tolerance[fatigue scale 14(FS-14)]and quality of life[the MOS item short from health survey(SF-36)]of the two groups were compared before and after the intervention.Result:Before intervention,there were no significant differences in lung function indexes between the two groups(P>0.05).After intervention,FVC and FEV1 in both groups were higher than those before intervention,and observation group were higher than those in control group,the differences were statistically significant(P<0.05).Before intervention,there were no significant differences in exercise endurance and fatigue tolerance between the two groups(P>0.05).After intervention,the 6MWT distance in both groups were longer than those before intervention,and the FS-14 score were lower than those before intervention,and the 6MWT distance in the observation group was longer than that in the control group,and the FS-14 score was lower than that in the control group,the differences were statistically significant(P<0.05).Before intervention,there was no significant difference in quality of life between the two groups(P>0.05).After intervention,the total SF-36 scores of both groups were higher than those before intervention,and the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:After pulmonary lobectomy,aerobic exercise training combined with respiratory rehabilitation training can effectively improve lung function,exercise endurance and fatigue tolerance,and improve quality of life.
