不同高度负跟鞋行走时腹背肌群及下肢肌群sEMG特征分析

Analysis of sEMG Characteristics of Abdominal and Dorsal Muscles and Lower Extremity Muscles during Walking with Different Heights of Negative Heel Shoes

摘要 采用横断面研究,选取健康人45例当受试者,让受试者分别穿上平底鞋及4种不同负跟高度的鞋子,测量其直线匀速行走时的双侧竖脊肌、腹直肌、胫骨前肌、腓肠肌内侧肌的均方根值,观察行走时5种鞋型下上述8块肌群的均方根值,并采用广义估计方程进行进一步分析。观察健康人穿平底鞋及负跟0.5、1.0、1.5、2.0 cm的鞋子对行走时双侧腰椎竖脊肌、腹直肌、胫骨前肌、腓肠肌内侧肌的均方根值,并对其进行进一步分析。结果表明双侧腹直肌、腰椎旁竖脊肌的均方根值随负跟高度的增加无明显规律性,双侧胫骨前肌的均方根值在负跟1.0 cm处有低峰,余负跟高度呈递增趋势,右侧腓肠肌内侧头的均方根值随着负跟高度增加呈递增趋势,左侧腓肠肌内侧头在负跟1.5 cm之前呈递增趋势,在负跟2.0 cm处呈下降趋势。因此得出,随着负跟高度的增加,对腹直肌、竖脊肌的影响无明显趋势变化;随着负跟高度的增加,对胫骨前肌、腓肠肌内侧头的影响出现趋势变化,其中对腓肠肌内侧头的影响更大,但均方根值并非随着的负跟高度的增加呈单调递增。 A cross-sectional study was used to select 45 healthy people as subjects. The subjects were asked to wear flat shoes and four shoes with different negative heel heights, and the bilateral erector spinae, abdominal muscles, and abdominal muscles were measured when they walked at a constant speed in a straight line. The root mean square value of the rectus muscle, tibialis anterior muscle, and medial gastrocnemius muscle was observed, and the root mean square value of the above 8 muscle groups under the five shoe types during walking was observed, and the generalized estimating equation was used for further analysis. We observed the root mean square of bilateral lumbar erector spinae, rectus abdominis, tibialis anterior and medial gastrocnemius when walking in healthy people wearing flat shoes and shoes with negative heels of 0.5 cm, 1.0 cm, 1.5 cm and 2.0 cm value and analyzed it. The results show that the root mean square value of bilateral rectus abdominis and lumbar paravertebral erector spinae has no obvious regularity with the increase of negative heel height. The negative heel height shows an increasing trend, the RMS value of the right medial head of gastrocnemius increases with the negative heel height, and the left medial head of gastrocnemius shows an increasing trend before the negative heel 1.5 cm and decreased at 2.0 cm of the negative heel trend. With the increase of negative heel height, there is no obvious trend change in the influence on rectus abdominis and erector spinal muscle. With the increase of negative heel height, the effect on the tibia anterior muscle and the medial head of the gastrocnemius muscle changes, of which the effect on the medial head of the gastrocnemius muscle is greater, but the mean square root value does not increase monotonically with the increase of the negative heel height.
