对比分析垂头综合征及颈椎病患者之间的颈椎矢状面排列差异

Contrastive analysis of differences in sagittal alignment of the cervical spine between patients with dropped head syndrome and cervical spondylosis

目的探讨分析垂头综合征(DHS)和颈椎病(CS)患者的颈椎矢状面排列情况。方法选取16例DHS患者作为观察组,另选取同期16例CS患者作为对照组。比较两组患者颈椎外侧站立X线摄影片的相关参数:C2~C7矢状面垂直轴距离(C2~7SVA)、CO~C2角(O~C2A)、C2斜率(C2S)、C2~C7角度(C2~7A),T_(1)斜率(T_(1)S)和C7矢状面垂直轴距离(C7SVA)。结果两组患者在O~C2A比较中差异无统计学意义(P>0.05)。DHS患者的C2~7SVA、T_(1)S和C2S明显大于CS患者,差异有统计学意义(P<0.05);而C2~7A、C7SVA明显小于CS患者,差异有统计学意义(P<0.05)。结论DHS患者的C2~7SVA、T_(1)S和C2S均明显大于CS,本文结果不仅在影像学方面能更好的区分DHS与CS,同时指出DHS的治疗方案应包括全脊柱整体矢状面排列。

Objective To explore the cervical sagittal alignment in patients with droopy head syndrome(DHS)and cervical spondylosis(CS).Methods Sixteen patients with DHS were selected as the observation group,and 16 patients with CS in the same period were included as the control group.The relevant parameters of the lateral standing radiographs of the cervical spine in the two groups were compared:the C2~C7 sagittal vertical axis distance(C2~7SVA),the CO~C2 angle(O~C2A),the C2 slope(C2S),the C2~C7 angle(C2~7A),the T_(1)slope(T_(1)S)and the C7 sagittal vertical axis distance(C7SVA).Results No significant difference was seen in the comparison of O~C2A between the two groups(P>0.05).C2~7SVA,T_(1)S,and C2S were significantly greater in DHS patients than in CS patients(P<0.05);whereas C2~7A and C7SVA were significantly smaller than in CS patients(P<0.05).Conclusion C2~7SVA,T_(1)S,and C2S are significantly greater in DHS patients than in CS.The present study not only better differentiates DHS from CS in terms of imaging,but also points out that the treatment plan for DHS should include overall sagittal alignment.
