摘要
选取符合纳入标准的100名社区高血压患者,随机分成观察组和对照组各50人。观察组采取的是为期6个月的团体健康管理签约服务方式,常规管理模式应用于对照组。通过分析慢性病团体健康管理签约服务模式应用于社区高血压患者中的干预效果,为高血压患者的社区健康管理提供理论和实践依据。
Selected 100 community hypertensive patients who meet the inclusion criteria and randomly divide them into observation group and control group of 50 people each.The observation group adopted 6-month group health management contract service method,and the conventional management model was applied to the control group.By analyzing the intervention effect of the chronic disease group health management contract service model applied to community hypertension patients,this provides theoretical and practical basis for community health management of hypertension patients.
