宣肺散邪分期辨治病毒性肺炎思路

Thoughts on Differentiation and Treatment of Viral Pneumonia by Stages of Xuanfei Sanxie

病毒性肺炎是一种传染性极强的呼吸系统疾病,病程发展急骤且近年来发病率处于上升趋势,中医分期辨治传染性疾病具有较好的理论和临床实践基础。本文从辨证、治疗两方面进行总结,并附验案具体阐释其临床应用。病毒性肺炎辨证时强调辨别湿、热、寒、毒、燥等不同邪气和热毒、痰浊、水湿、瘀血、虚损等不同证候要素,关键病机为温邪袭肺、肺气郁闭,治疗当以宣肺散邪为核心,针对初期、中期、重症期、恢复期等不同分期,灵活运用透、清、化、下等多种散邪方法,以经方药为基础加减进行辨治,为临床治疗提供借鉴。

Viral pneumonia is a highly contagious respiratory disease with a rapid course of disease and an increasing incidence in recent years.And there is a good theoretical and clinical basis for the staged treatment of infectious diseases in Chinese medicine.This article summarizes the syndrome differentiation and treatment,and attaches the case to explain its clinical application.It is pointed out that the syndrome differentiation of viral pneumonia emphasizes the identification of different evil Qi such as dampness,heat,cold,poison and dryness and different syndrome factors such as heat toxin,phlegm turbidity,water dampness,blood stasis and asthenic disease,and the key pathogenesis is the invasion of lung by warm evil and stagnation of lung Qi.When the treatment takes Xuanfei Sanxie as the core,according to different stages such as the initial stage,the middle stage,the severe stage and the recovery stage,a variety of dispelling evil methods such as penetration,clearing,dissipation and purgative method are flexibly used.The treatment is based on the classical prescriptions with the add and subtract,so as to provide reference for clinical treatment.
