子宫内膜中CD16^(+)CD69^(+)NK细胞百分比的增加与不孕症的相关性分析

The correlation annalysis between the increase of the percentagee of CD16^(+)CD69^(+)uNK cells in endometrium and infertility

摘要 目的分析子宫内膜自然杀伤(natural killer,NK)细胞与女性不明原因不孕症(unexplained infertility,UI)的关系。方法随机选取并收集2022年10月至2023年1月在吉林大学第二医院行宫腔镜手术的不孕症女性部分子宫内膜组织作为研究组,并随机收集同期健康已婚已育女性的适量子宫内膜组织作为对照组,使用流式细胞术检测子宫内膜中CD56+CD3-NK细胞和CD16^(+)CD69^(+)NK细胞的百分比,采用SPSS 26.0、Medcalc统计学软件对数据进行分析。结果(1)两组患者的年龄、月经周期等一般临床资料比较,差异均无统计学意义(P>0.05)。(2)研究组与对照组CD56+CD3-NK细胞的百分比为[20.42%(13.63%,41.58%)vs 8.20%(5.83%,9.15%)],研究组与对照组CD16^(+)CD69^(+)NK细胞的百分比为[45.30%(22.20%,72.83%)vs 10.75%(3.58%,17.28%)],差异有统计学意义(P<0.001)。(3)CD16^(+)CD69^(+)NK(%)诊断UI时的曲线下面积(area under the curve,AUC)为0.862(95%CI:0.735-0.943),灵敏度为78.95%,特异度为100.00%(P<0.001)。结论子宫内膜中CD16^(+)CD69^(+)NK(%)细胞百分比的增加与不孕症的发生具有相关性,它的上调预示着某些患者局部免疫的激活,可以将其作为一个治疗靶点。 Objective To analyze the relationship between uterine natural killer(NK)cells and female unexplained infertility(UI).Methods Some endometrial tissues were randomly selected and collected from women with infertility who underwent hysteroscopic surgery at the Second Norman Bethune Hospital of Jilin University from October 2022 to January 2023,and an appropriate amount of endometrial tissues were randomly collected from healthy married and fertile women during the same period,and the percentages of CD56*CD3'NK cells and CD16^(+)CD69^(+)NK cells in the endometrium were detected using flow cytometry.SPSS 26.0 and Medcalc statistical sofwares were used to analyze the data.Results There was no significant difference in age,menstrual cycle and other general clinical data beween the two groups(P>0.05);②The percentage of CD56^(+)CD3^(-)NK cells in study group and control group was[20.42%(13.63%,41.58%)vs 8.20%(5.83%,9.15%)],the percentage of CD16^(+)CD69^(+)NK cells in the study group and the control group was[45.30%(22.20%,72.83%)vs 10.75%(3.58%,17.28%)],and the difference was statistically significant(P<0.001).③The area under the curve(AUC)for CD16^(+)CD69^(+)NK(%)in diagnosing UI was 0.862(95%Cl:0.735-0.943),the sensitivity was 78.95%,and the specificity was 100.00%(P<0.001).Conclusion There is a correlation between the increased percentage of CD16^(+)CD69^(+)NK cells in the endometrium correlates with occurrence of infertility,and its upregulation predicts the activation of local immunity in certain patients with UI,which can be used as a therapeutic target.
