摘要
目的探讨血清微小RNA(miR)-203与miR-221表达水平在胰腺癌早期诊断中的应用价值。方法选取本院2021年1月至2022年6月收治的108例胰腺癌患者为研究组,选取同时期本院收治的106例胰腺良性疾病患者为良性组,同时期106例健康体检者为对照组,采用实时荧光定量聚合酶链式反应(qRT-PCR)检测三组受试者的血清miR-203与miR-221表达水平。采用Logistic回归分析胰腺癌发病的影响因素;通过受试者工作特征(ROC)曲线分析血清miR-203与miR-221表达水平对胰腺癌的诊断价值。结果研究组、良性组、对照组患者的血清miR-203与miR-221表达水平依次降低(P<0.05)。胰腺癌患者血清miR-203、miR-221表达水平与肿瘤直径、TNM分期、淋巴结转移、分化程度相关(P<0.05)。多因素Logistic回归分析结果表示胰腺癌家族史、血清miR-203与miR-221表达水平均为胰腺癌发病的危险因素(P<0.05)。血清miR-203与miR-221水平联合诊断胰腺癌的ROC曲线下面积为0.878(95%CI:0.826~0.918),大于单独miR-203诊断的AUC(Z=2.229,P=0.026)和单独miR-221诊断的AUC(Z=2.311,P=0.021)。结论胰腺癌患者血清miR-203、miR-221均异常高表达,两者在胰腺癌早期诊断中均具有应用价值,且二者联合的诊断效能更好。
Objective To investigate the applieation value of serum miR-203 and miR-221 expression levels in the early diag-nosis of pancreatic cancer.Methods 108 patients with pancreatic cancer admitted to our hospital from January 2021 to June 2022 were selected as the study group,and 106 patients with benign pancreatic diseases admitted to our hospital during the same period were selected as the benign group,and 106 health examiners at the same time were selected as the control group.Real-time fluorescence quantitative polymerase chain reaction(qRT-PCR)was applied to detect the expression levels of serum miR-203 and miR-221 in the three groups.Logistic regression was applied to analyze the influencing factors of occurrence of pancreatic cancer,and receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of serum miR-203 and miR-221 expression levels for pancreatic cancer.Results The expression levels of serum miR-203 and miR-221 in the study group,benign group,and control group decreased sequentially(P<0.05).The expression levels of serum miR-203 and miR-221 in pancreatic cancer patients were correlated with tumor diameter,TNM stage,lymph node metastasis and differentiation(P<0.05).Multivariate Logistic regression analysis showed that family history of pancreatic cancer,serum miR-203 and miR-221 expression levels were risk factors for pancreatic cancer(P<0.05).The area under the ROC curve(AUC)for the combined diagnosis of serum miR-203 and miR-221 levels of pancreatic cancer was 0.878(95%CI:0.826~0.918),which was larger than AUC diagnosed by miR-203 alone(Z=2.229,P=0.026)and AUC diagnosed by miR-221 alone(2=2.311,P=0.021).Conclusion Serum miR-203 and miR-221 are abnormally high expressed in pancreatie cancer patients.Both of them have practical value in the early diagnosis of pancreatic cancer,and their combined diagnstie effivacy is beller.
