标准化脑膜中动脉栓塞治疗高危复发性慢性硬膜下血肿疗效观察

Efficacy Observation of Standardized Middle Meningeal Artery Embolization for High-risk Recurrent Chronic Subdural Hematoma

目的:观察并分析脑膜中动脉栓塞治疗高危复发性慢性硬膜下血肿的有效性,并对该治疗方法做安全性分析。方法:回顾性分析23例2020年3月至2022年3月在重庆大学附属三峡医院就医并行脑膜中动脉栓塞治疗的慢性硬膜下血肿患者,其中20例单侧复发性慢性硬膜下血肿患者,3例双侧复发性慢性硬膜下血肿患者,分析脑膜中动脉栓塞术后一个月、三个月及六个月患者血肿复发和进展情况。统计治疗的有效性结果及安全性。结果:观察组患者治疗之后复发率为0,明显低于对照组25.00%,差异有统计学意义,(P<0.05);两组患者治疗有效率对比差异无统计学意义,(P>0.05)。结论:脑膜中动脉栓塞治疗可明显改善慢性硬膜下血肿患者的治疗效果,并降低复发率,值得临床进一步研究。

Objective:To observe and analyze the efficacy of middle meningeal artery embolization in the treatment of high-risk recurrent chronic subdural hematoma,and analyze its safety.Methods:A retrospective analysis was performed on 23 patients with chronic subdural hematoma who were treated by middle meningeal artery embolization in Three Gorges Hospital Affiliated to Chongqing University from March 2020 to March 2022,including 20 patients with unilateral recurrent chronic subdural hematoma and 3 patients with bilateral recurrent chronic subdural hematoma.The blood recurrence and progression of patients at 1 month,3 months and 6 months after middle meningeal artery embolization were analyzed.The efficacy,outcome and safety of the treatment were analyzed.Results:The recurrence rate of the observation group after treatment was 0,which was significantly lower than that of the control group by 25.00%,with statistical significance(P<0.05).There was no significant difference in treatment efficiency between the two groups(P>0.05).Conclusion:Middle meningeal artery embolization can significantly improve the therapeutic effect of chronic subdural hematoma and reduce the recurrence rate,which is worthy of further clinical study.
