正中神经针灸电刺激与正中神经经皮电刺激促醒疗效对比研究

Comparison of the Effects of Median Nerve Acupuncture Electric Stimulation and Median Nerve Percutaneous Electrical Stimulation on Awakening

目的:观察正中神经针灸电刺激对颅脑创伤后昏迷患者的促醒作用,并和正中神经经皮电刺激的疗效进行比较。方法:纳入颅脑创伤后昏迷患者30例,随机分配到正中神经针灸电刺激组(电针组)、正中神经经皮电刺激组(MNS组)和对照组,每组各10例,3组均接受常规促醒治疗,电针组在常规治疗基础上接受正中神经针灸电刺激治疗,MNS组在常规治疗基础上增加正中神经经皮电刺激。在干预前后采用格拉斯哥昏迷量表(GCS)和改良昏迷恢复量表(CRS-R)对3组受试者的意识水平进行评估。结果:干预后,电针组GCS总分优于MNS组和对照组,差异有统计学意义(P<0.05);电针组CRS-R总分及其子项目评分均优于对照组,差异有统计学意义(P<0.05)。结论:正中神经针灸电刺激对颅脑损伤昏迷患者具有积极的促醒作用,对意识水平改善情况可能略优于正中神经经皮电刺激。

Objective:To observe the effect of median nerve acupuncture electric stimulation on awakening of coma patients after traumatic brain injury,and compare the effect with that of median nerve percutaneous electrical stimulation.Methods:Thirty patients with post-traumatic coma who were admitted to the Third Hospital of Xiamen from May 2019 to April 2022 were randomly assigned to the median nerve acupuncture electric stimulation group(electroacupuncture group),the median nerve percutaneous electrical stimulation group(MNS group)and the control group,with 10 patients in each group.All three groups received conventional wake-up therapy.The electroacupuncture group received the median nerve acupuncture electric stimulation on the basis of conventional treatment.In MNS group,median nerve percutaneous electrical stimulation was added to conventional treatment.Before and after the intervention,Glasgow Coma Scale(GCS)and Modified Coma Recovery Scale(CRS-R)were used to evaluate the consciousness level of the three groups.Results:After intervention,the total score of GCS in the electroacupuncture group was superior to that in the MNS group and the control group,with a significant difference(P<0.05).The total score of CRS-R and its sub-items in the electroacupuncture group were superior to that in the control group,with a significant difference(P<0.05).Conclusion:The median nerve acupuncture electric stimulation has a positive effect on awakening of coma patients with traumatic brain injury,and its improvement of consciousness level may be slightly better than the median nerve percutaneous electrical stimulation.
