活血止痛汤联合手法复位小夹板固定对桡骨远端骨折术后疼痛及腕关节功能的影响

Effect of Huoxue Zhitong Decoction combined with manual reduction and small splint fixation on postoperative pain and wrist function in patients with distal radius fracture

目的观察活血止痛汤联合手法复位小夹板固定对桡骨远端骨折患者术后疼痛及腕关节功能的影响。方法选取靖安县中医院收治的82例桡骨远端骨折患者,选取年限为2020年6月至2022年8月,随机分为两组,即对照组(n=41)和观察组(n=41),对照组给予手法复位小夹板固定治疗,观察组在此基础上联合活血止痛汤治疗1个月;于治疗前、治疗14 d、治疗1个月时,比较两组疼痛度[视觉模拟评分(VAS)]及治疗前后腕关节功能和活动度。结果治疗14 d、1个月时,两组VAS评分较治疗前下降,且观察组低于对照组,差异有统计学意义(P<0.01);治疗后,两组腕关节功能优良率及腕关节背伸、掌屈、旋前、旋后活动度均较治疗前提高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论在外固定术基础上,活血止痛汤可减轻桡骨远端骨折患者术后疼痛感,提高腕关节功能及活动度,具有一定应用价值。

Objective To observe the effects of Huoxue Zhitong Decoction combined with manual reduction and small splint fixation on postoperative pain and wrist function in patients with distal radius fracture.Methods A total of 82 patients with distal radius fracture admitted to Jing'an Hospital of Traditional Chinese Medicine from June 2020 to August 2022 were randomly divided into two groups:control group(n=41)and observation group(n=41).The control group were treated with manual reduction and small splint,and based on this,the observation group were treated with Huoxue Zhitong Decoction for 1 month.And then,pain degrees(visual analog scale[VAS])before treatment,14 days and 1 month after treatment as well as wrist function and range of motion before and after treatment were compared between the two groups.Results After 14 days and 1 month of treatment,VAS scores of the two groups were lower than those before treatment,those of the observation group were lower than those of the control group,and difference was statistically significant(P<0.01).After treatment,the excellent and good rate of wrist joint function as well as the motion range of wrist joint dorsiflexion,palm flexion,pronation,and supination in both groups were higher than those before treatment,and those of the observation group were higher than those of the control group,so difference was statistically significant(P<0.05).Conclusion On the basis of fixation,Huoxue Zhitong Decoction can reduce postoperative pain in patients with distal radius fracture and improve wrist joint function and range of motion,which has certain application value.
