Effect of cluster management strategy on nosocomial infection control

摘要 目的探讨集束化感控管理模式对医院感染预防控制的效果。方法选取2020年1月—2021年1月作为干预前组,自2021年1月开始将集束化感控管理模式应用于感染防控工作,并在实施1年中持续进行质量改进,将2021年1—12月作为干预后组,比较干预前后住院患者医院感染控制的各项指标变化情况。结果干预后医院感染率下降(x^(2)=13.842,P<0.05),CLABSI千日感染率和CAUTI千日感染率降低(x^(2)=4.370,P<0.05;x^(2)=15.082,P<0.001),多重耐药菌感染发现率、感染检出率均显著降低(x^(2)=4.799,P<0.05;x^(2)=31.676,P<0.05),手术部位感染率从0.20%降低至0.13%(x^(2)=4.690,P=0.030),手卫生依从率提高至80.16%(x^(2)=369.209,P<0.001),手卫生正确率提高至79.35%(x^(2)=507.178,P<0.001)。结论集束化感控管理模式能够显著降低医院感染风险,提升医院感染管理质量。 Objective To explore the effect of cluster management strategy on nosocomial infection control.Methods The pre-intervention group from January 2020 to January 2021 was selected,and the cluster management strategy was applied from January 2021.The quality improvement was carried out during one year.From January to December 2021 was the post-intervention group.The changes of various indicators of nosocomial infection control in inpatients before and after the intervention were compared.Results The incidence of nosocomial infection declined generally after intervention(x^(2)=13.842,P<0.05).The thousand day infection rate of CLABSI and CAUTI were decreased(CLABSIx^(2)=4.370,P<0.05;CAUTIx^(2)=15.082,P<0.001),and the nosocomial infection rate of MDRO and the detection rate of MDRO in the whole hospital were decreased(x^(2)=4.799,P<0.05;the detection rate of MDRO,x^(2)=31.676,P<0.05).In addition,the surgical site infection rate was reduced from 0.20%to 0.13%(x^(2)=4.690,P=0.030),the compliance rate of hand hygiene increased to 80.16%(x^(2)=369.209,P<0.001),and the correct rate of hand hygiene was increased to 79.35%(x^(2)=507.178,P<0.001).Conclusion Cluster management strategy can significantly reduce the risk of nosocomial infection and improve the quality of hospital infection control.
