某医院多部门联动控制多重耐药菌感染效果观察

Effect of multi-department interaction on preventing multidrug-resistant organism infection in a hospital

摘要 目的探讨某医院多部门联动遏制多重耐药菌(MDRO)感染的作用。方法以某医院2017年6月1日—2019年6月31日和2019年7月1日—2021年6月31日入院的患者资料为研究对象,分别为对照组和观察组,对照组采用常规防控措施,观察组采用多部门联动落实防控措施。分析2组MDRO感染现状、抗菌药物使用强度、防控措施落实依从性、MDRO住院日医院感染率、MDRO社区定植率和医院定植率。结果观察组MDRO检出次数、例次数、感染类型等感染现状与对照组差异无统计学意义。观察组抗菌药物累计每100人日消耗抗菌药物的限定日剂量(DDD)数、总住院天数低于对照组,分别为(5124.25±101.01)和(4387.24±124.36)d;抗菌药物使用强度、防控措施落实依从性、平均住院天数高于对照组(P<0.05)。观察组MDRO住院日医院感染率、MDRO社区定植率和医院定植率均低于对照组(P<0.05)。结论多部门联动落实防控措施可提高临床医务人员MDRO防控措施的落实依从性,促进抗菌药物合理应用,降低MDRO医院感染发生率。 Objective To investigate the role of multi-departmental linkage to contain multi-drug resistant organism(MDRO)infections in a hospital.Methods Patients admitted to a hospital from June 1,2017 to June 31,2019 and from July 1,2019 to June 31,2021 were selected for control group and obvservation group,respectively.The control group was treated with conventional preventive and control measures and the observation group were treated with multi-departmental linkage measures.The current infection status of MDRO in the 2 groups,intensity of antimicrobial drug use,compliance with the implementation of prevention and control measures,hospital infection rate of MDRO on hospital days,community colonization rate and hospital colonization rate of MDRO were analyzed.Results There was no statistical difference between the current infection status of the experimental group and the control group in terms of the number of MDRO detected,the number of cases and the type of infection.The cumulative number of limited daily doses(DDD)of antimicrobial drugs consumed per 100 person-days and total hospital days were lower in the experimental group than in the control group,(5124.25±101.01)and(4387.24±124.36)d,respectively,and the intensity of antimicrobial drug use,compliance with implementation of preventive and control measures,and mean hospital days were higher in the experimental group than those in the control group(P<0.05).The hospital infection rate of MDRO'inpatient days,community colonization rate and hospital colonization rate of MDRO in the experimental group were lower than those in the control group(P<0.05).Conclusion Multi-departmental joint implementation of preventive and control measures can improve the compliance of clinical medical staff in implementing preventive and control measures for MDRO and can promote the rational application of antibiotics,which can reduce the incidence of MDRO hospital infection.
