残余胆固醇与糖尿病视网膜病变相关性的研究

Correlation analysis of residual cholesterol with diabetic retinopathy

目的探讨残余胆固醇(RC)与DR的相关性。方法选取2018年3月至2022年9月于徐州医科大学附属医院内分泌科住院治疗的T2DM患者415例,根据眼底照相结果分为单纯T2DM组(n=197)和DR组(n=218)。比较各组一般资料及生化指标。结果与T2DM组比较,DR组DM病程、BMI、高血压病比例、HbA_(1)c、TC、LDL-C、RC、纤维蛋白原(FIB)升高(P<0.05),HDL-C降低(P<0.05)。Pearson相关分析显示,RC与BMI、FPG、HbA_(1)c、TC、TG、FIB呈正相关(P<0.05),与年龄、HDL-C呈负相关(P<0.05)。Logistic回归分析显示,DM病程、高血压病、HbA_(1)c、RC、FIB是DR的影响因素。受试者工作特征(ROC)曲线分析显示,RC的ROC曲线下面积低于HbA_(1)c,敏感度为84.9%,特异度为54.8%。结论RC与不同程度DR相关,是DR的影响因素,可预测DR。

Objective To investigate the correlation between residual cholesterol(RC)and diabetic retinopathy(DR).Methods A total of 415 patients with type 2 diabetes mellitus(T2DM)were enrolled in this study from the Endocrinology Department of Xuzhou Medical University Affiliated Hospital from March 2018 to September 2022.All the patients were divided into simple T2DM group(n=197)and DR group(n=218)according to fundus photographic results.The general information and biochemical indicators were compared between the two groups.Results Compared with T2DM group,DM duration,the proportion of hypertension,RC,low-density lipoprotein(LDL-C),fibrinogen(FIB),total cholesterol(TC),HbA_(1)c,BMI and high-density lipoprotein(HDL-C)were significantly increased in DR group(P<0.05).Pearson correlation analysis showed that RC was positively correlated with BMI,FPG,HbA_(1)c,TC,TG and FIB(P<0.05),while negatively correlation with age and HDL-C(P<0.05).Logistic regression analysis showed that RC,HbA_(1)c,DM duration,hypertension,and FIB were the influencing factors for DR.The receiver operating characteristic curve(ROC)showed that the area under the ROC curve of RC was smaller than HbA_(1)c,with a sensitivity of 84.9%and a specificity of 54.8%.Conclusion RC is correlated with different degrees of DR,and RC is an influencing factor and also a predictor for DR.
