麦默通微创旋切手术治疗良性乳腺肿瘤的临床疗效分析

Clinical efficacy analysis of Mammotome minimally invasive surgery for benign breast tumors

目的探索麦默通微创旋切手术治疗良性乳腺肿瘤的临床疗效。方法100例良性乳腺肿瘤患者,根据手术方案不同分为对照组和观察组,每组50例。对照组采用开放式手术治疗,观察组采用麦默通微创旋切手术治疗。比较两组患者手术指标(手术时间、术中出血量、并发症发生情况、切口愈合时间、住院时间、乳腺外形良好率)及术后炎症指标[肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)]、免疫指标(CD3^(+)、CD4^(+)/CD8^(+))。结果观察组患者手术时间(17.59±8.12)min、切口愈合时间(4.07±1.31)d、住院时间(1.23±0.21)d均短于对照组的(29.12±11.34)min、(5.56±1.71)d、(4.45±0.67)d,术中出血量(23.12±7.64)ml少于对照组的(54.45±17.59)ml,并发症发生率2.00%(1/50)低于对照组的16.00%(8/50),乳腺外形良好率100.00%(50/50)高于对照组的80.00%(40/50),差异均有统计学意义(P<0.05)。术后,观察组患者TNF-α(1.56±0.21)ng/L、hs-CRP(7.19±1.02)mg/L低于对照组的(3.75±0.34)ng/L、(13.25±3.23)mg/L,CD3+(78.91±5.21)%、CD4^(+)/CD8^(+)(1.98±0.26)高于对照组的(53.11±3.40)%、(1.13±0.12),差异均有统计学意义(P<0.05)。结论麦默通微创旋切手术可以明显降低良性乳腺肿瘤患者的外科创伤,减少术后并发症,同时也降低了对患者机体免疫系统的损害,从而促进了患者的术后康复,提高了患者乳房外观良好率,值得临床推广应用。

Objective To analyze the clinical efficacy of Mammotome minimally invasive surgery for benign breast tumors.Methods A total of 100 patients with benign breast tumor were divided into a control group and an observation group according to different surgical regimens,with 50 cases in each group.The control group was treated with open surgery,and the observation group was treated with Mammotome minimally invasive surgery.Both groups were compared in terms of surgical indexes(operative time,intraoperative blood loss,complications,incision healing time,hospital stay,rate of good breast appearance),postoperative inflammatory indexes[tumor necrosis faction-α(TNF-α),hypersensitive C-reactive protein(hs-CRP)],and immune indexes(CD3^(+),CD4^(+)/CD8^(+)).Results The observation group had operative time of(17.59±8.12)min,incision healing time of(4.07±1.31)d and hospital stay of(1.23±0.21)d,which were shorter than those of(29.12±11.34)min,(5.56±1.71)d and(4.45±0.67)d in the control group;the intraoperative blood loss of(23.12±7.64)ml in the observation group was less than that of(54.45±17.59)ml in the control group;the complication rate of 2.00%(1/50)in the observation group was lower than that of 16.00%(8/50)in the control group;the rate of good breast appearance of 100.00%(50/50)in the observation group was higher than that of 80.00%(40/50)in the control group;the differences were statistically significant(P<0.05).After surgery,TNF-αof(1.56±0.21)ng/L and hs-CRP of(7.19±1.02)mg/L in the observation group were lower than those of(3.75±0.34)ng/L and(13.25±3.23)mg/L in the control group;CD3+of(78.91±5.21)%and CD4^(+)/CD8^(+)of(1.98±0.26)in the observation group were higher than those of(53.11±3.40)%and(1.13±0.12)in the control group;the differences were statistically significant(P<0.05).Conclusion Mammotome minimally invasive surgery can significantly reduce the surgical trauma of patients with benign breast tumors,reduce postoperative complications,and also reduce the damage to the patient's immune system,thereby promoting the postoperative recovery of patients and improving the rate of good breast appearance.It is worthy of clinical promotion.
