盆底超声联合腹直肌超声在女性漏尿症病因诊断中的临床应用研究

Clinical application of pelvic floor ultrasonography combined with rectus abdominis ultrasonography in the etiological diagnosis of female urinary leakage

目的:观察盆底超声检查联合腹直肌超声检查在女性漏尿病因诊断中的应用效果。方法:选取2020年3月—2021年12月收治的女性漏尿症患者40例作为观察组,选取同期接受健康检查的女性40例作为对照组。两组均采用盆底超声联合腹直肌超声检查,比较两组静息状态下和最大Valsalva动作下的膀胱尿道后角、尿道倾斜角、膀胱颈移动度;比较两组腹直肌超声静息和卷腹状态下脐水平、脐上3~5 cm及脐下3~5 cm水平的腹直肌间距;比较两组尿道内口形成情况。结果:两组女性静息状态下尿道倾斜角、膀胱尿道后角和膀胱颈移动度比较,差异均无统计学意义(P>0.05);最大Valsalva动作下观察组尿道倾斜角、膀胱尿道后角和膀胱颈移动度均高于对照组,差异有统计学意义(P<0.05)。静息状态和卷腹状态下观察组腹直肌间距均大于对照组,差异有统计学意义(P<0.05)。观察组尿道内口漏斗形成率高于对照组,差异有统计学意义(P<0.05)。结论:采用盆底超声联合腹直肌超声检查可评估女性漏尿症患者病情程度,真实、准确地反映出患者的腹直肌及盆底结构与功能并能预测疗效,为临床决策、干预提供可靠信息。

Objective:To observe the effect of pelvic floor ultrasonography combined with rectus abdominis ultrasonography in the etiological diagnosis of female urinary leakage.Methods:ALL 40 female patients with urinary leakage admitted to hospital from March 2020 to December 2021 were selected as the observation group,and 40 female patients receiving health examination during the same period were selected as the control group.Both groups of women were examined by pelvic floor ultrasound combined with rectus abdominis ultrasound.The posterior Angle of bladder and urethra,urethral inclination Angle and bladder neck mobility of the two groups were observed and compared in the resting state and the maximum Valsalva operation,and the distance between rectus abdominis muscles at the umbilical level,3~5 cm above the umbilical level and 3~5 cm below the umbilical level were compared in the resting state and the rolling state of rectus abdominis muscles,and the formation of the internal urethra opening of the two groups was compared.Results:There were no significant differences in urethral inclination Angle,posterior Angle of bladder urethra and bladder neck mobility between the two groups at rest(P>0.05),and the observation group under maximum Valsalva movement was higher than the control group(P<0.05).The distance between rectus abdominis muscle in the observation group was greater than that in the control group in the resting state and the rolling state(P<0.05).The formation rate of funnel in the observation group was higher than that in the control group(P<0.05).Conclusion:Pelvic floor ultrasound combined with rectus abdominis ultrasound can evaluate the degree of urinary incontinence patients,reflect the structure and function of rectus abdominis and pelvic floor of patients truly and accurately,and predict the efficacy,providing reliable information for clinical decision making and intervention.
