摘要
目的探讨乳腺癌患者血清癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原153(CA153)、糖类抗原199(CA199)的表达及其对肿瘤转移的预测价值。方法抽取驻马店市中心医院2021年12月至2022年12月收治的64例乳腺癌患者作为研究对象,根据临床表现及影像学、病理检查等确诊为肿瘤转移患者设为肿瘤转移组(27例),未发生肿瘤转移者设为肿瘤未转移组(37例),另抽取同期健康志愿者作为对照组(30例)。采用化学发光法检测三组血清CEA、CA125、CA153、CA199水平;采用受试者工作特征曲线分析血清CEA、CA125、CA153、CA199对乳腺癌患者发生肿瘤转移的诊断价值。结果肿瘤转移组原发肿瘤长径>5 cm者及组织学分级Ⅲ级者占比高于肿瘤未转移组(P<0.05)。三组血清CEA、CA125、CA153、CA199比较差异有统计学意义(P<0.05),肿瘤转移组、肿瘤未转移组血清CEA、CA125、CA153、CA199水平均高于对照组,且肿瘤转移组高于肿瘤未转移组(P<0.05)。采用受试者工作特征曲线分析结果显示,血清CEA、CA125、CA153、CA199的曲线下面积分别为0.743(95%CI:0.615~0.871)、0.735(95%CI:0.609~0.861)、0.762(95%CI:0.647~0.878)、0.699(95%CI:0.567~0.831),灵敏度分别为70.4%、55.6%、63.0%、55.6%,特异度分别为78.4%、86.5%、75.7%、73.0%。结论乳腺癌患者血清CEA、CA125、CA153、CA199呈现高表达水平,血清CEA、CA125、CA153、CA199水平对乳腺癌患者发生肿瘤转移具有一定的诊断效能,可作为诊断乳腺癌患者发生肿瘤转移的血清标志物。
Objective To investigate the expression of serum carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),carbohydrate antigen 153(CA153)and carbohydrate antigen 199(CA199)in patients with breast cancer,and their diagnostic value for tumor metastasis.Methods Sixty-four patients with breast cancer treated in Zhumadian Central Hospital from December 2021 to December 2022 were selected.Among them,patients who were diagnosed with tumor metastasis by clinical manifestations,imaging examination and pathological examination were allocated into metastasis group(27 cases),and patients without tumor metastasis were allocated into non-metastasis group(37 cases).Another 30 healthy volunteers during the same period were selected as control group(30 cases).The levels of CEA,CA125,CA153 and CA199 in serum of the three groups were detected by chemiluminescence assay.Then,receiver operating characteristic curve was plotted to evaluate the diagnostic value of serum CEA,CA125,CA153 and CA199 for the occurrence of tumor metastasis in breast cancer patients.Results The proportion of primary tumor with major diameter>5 cm and histological grade III in the metastasis group was higher than that in the non-metastasis group(P<0.05).There were significant differences in levels of serum CEA,CA125,CA153 and CA199 among the three groups(P<0.05),while the levels of serum CEA,CA125,CA153 and CA199 in the metastasis group and non-metastasis group were higher than those in the control group,and the levels in the metastasis group were higher than those in the non-metastasis group(P<0.05).Results of receiver operating characteristic curve showed that the area under the curve of serum CEA,CA125,CA153 and CA199 were 0.743(95%CI:0.615-0.871),0.735(95%CI:0.609-0.861),0.762(95%CI:0.647-0.878)and 0.699(95%CI:0.567-0.831);sensitivity of them were 70.4%,55.6%,63.0%and 55.6%,respectively;and specificity of them were 78.4%,86.5%,75.7%and 73.0%,respectively.Conclusions Serum CEA,CA125,CA153 and CA199 have high expression level in breast cancer patients,and the four indicators have certain diagnostic efficacy for tumour metastasis in breast cancer patients,therefore,they all can be used as serum markers for the diagnosis of tumour metastasis in breast cancer patients.
