摘要
目的探讨外周血单核细胞CD169和血清干扰素-γ(IFN-γ)联合检测对小儿病毒性感染的早期诊断价值。方法选取2021年1月—2022年6月该院收治的感染性100例小儿患者,根据微生物学或分子生物学检测结果,将小儿患者分为病毒感染组(62例)与细菌感染组(38例),同时收集50例健康小儿标本作为参照组。使用流式细胞法检测各组患儿外周血单核细胞CD169表达情况,酶联免疫吸附法检测血清IFN-γ水平,受试者工作特征(ROC)曲线比较单核细胞CD169、IFN-γ的评估价值。结果病毒感染组、细菌感染组单核细胞CD169表达水平高于参照组,且细菌感染组单核细胞CD169表达水平高于病毒感染组(P<0.05)。病毒感染组、细菌感染组IFN-γ水平高于参照组,且细菌感染组IFN-γ水平高于病毒感染组(P<0.05)。根据ROC曲线,两项联合检测的曲线下面积值为0.954,高于单核细胞CD169、IFN-γ单一检测0.879、0.840(P<0.05)。结论在小儿病毒性感染诊断中,外周血单核细胞CD169和血清IFN-γ联合检测具有重要作用,有助于早期发现病毒性感染,并给予积极干预措施,可作为诊断小儿病毒性感染的重要指标。
Objective To explore the value of combined detection of peripheral blood monocyte CD169 and serum cellsγ-Interferon(IFN-γ)in early diagnosis of viral infection in children.Methods A total of 100 infectious children admitted to the hospital from January 2021 to June 2022 were selected.According to the microbiological or molecular biological test results,the children were divided into two groups:a group with viral infection(62 cases)and a group with bacterial infection(38 cases).At the same time,50 healthy children samples were collected as a reference group.Use this flow cytometry to detect the expression of CD169 in peripheral blood mononuclear cells of children in each group,and use enzyme-linked immunosorbent assay to detect the serum level of IFN-γconcentration,Receiver operating characteristic(ROC)curve was used to compare the value of monocyte CD169,this IFN-γ.Results The expression level of CD169 on monocytes in the virus infected group and the bacterial infected group was higher than that in the reference group,and the expression level of CD169 on monocytes in the bacterial infected group was higher than that in the virus infected group(P<0.05).Virus infection group,this bacterial infection group IFN-γThe concentration is higher than that of the reference group,and the interferon IFN in the bacterial infection group-γThe concentration was higher in the group infected with EHV(P<0.05).According to the ROC curve,the area under the ROC value of the two combined tests is 0.954,which is higher than the monocyte CD169 and IFN-γ,Single detection was 0.879 and 0.840(P<0.05).Conclusion In the diagnosis of viral infection in children,peripheral blood monocyte cells CD169 and serum IFN-γ.Joint testing has this important role,helping to detect this viral infection early,and can be used as an important indicator for diagnosing this viral infection in children.
