尿动力学检查诊断女性压力性尿失禁的价值分析

Analysis on Value of Urodynamic Examination in Diagnosis of Stress Urinary Incontinence in Women

目的:探讨尿动力学检查诊断女性压力性尿失禁的价值。方法:选取2020年1月—2022年12月赤壁市蒲纺医院收治的71例压力性尿失禁患者作为观察组,同期选取50例健康体检者作为对照组。根据Stamey分级将观察组分为Ⅰ级组、Ⅱ级组、Ⅲ级组,观察组与对照组均行尿动力学检查。比较观察组与对照组尿动力学检查参数指标,分析压力性尿失禁分级与尿动力学检查参数的相关性。结果:观察组最大逼尿肌压力、最大尿道关闭压、半充盈压水平低于对照组,差异有统计学意义(P<0.001)。不同Stamey分级患者最大逼尿肌压力、最大尿道关闭压、半充盈压水平比较,Ⅰ级组>Ⅱ级组>Ⅲ级组,差异有统计学意义(P<0.05)。Pearson分析显示,最大逼尿肌压力、最大尿道关闭压、半充盈压水平与Stamey分级呈负相关(r=-0.430、-0.433、-0.482,P<0.05)。结论:尿动力学检查在女性压力性尿失禁的诊断中有一定价值,能显示患者病情严重程度,直观、量化反映膀胱、尿道功能,患者病情越重,最大逼尿肌压力、最大尿道关闭压、半充盈压水平越低。

Objective:To investigate the value of urodynamic examination in diagnosis of stress urinary incontinence in women.Methods:A total of 71 patients with stress urinary incontinence admitted to Chibi City Pufeng Hospital from January 2020 to December 2022 were selected as the observation group,and 50 people for check-up at the same period were selected as the control group.The observation group was divided into grade I group,grade II group,and grade III group according to Stamey grade.Urodynamic examination was performed in the observation group and the control group.The urodynamic examination parameters in the observation group and the control group were compared.The correlation between stress urinary incontinence grade and urodynamic examination parameters was analyzed.Results:The levels of maximum detrusor pressure,maximum urethral closure pressure and semi-filling pressure in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.001).The levels of maximum detrusor pressure,maximum urethral closure pressure and semi-filling pressure were ranked as follows:grade I group>grade II group>grade III group,and there were statistically significant differences(P<0.05).Pearson analysis showed that,the levels of maximum detrusor pressure,maximum urethral closure pressure and semi-filling pressure were negatively correlated with Stamey grade(r=-0.430,-0.433,-0.482,P<0.05).Conclusion:Urodynamic examination has a certain value in the diagnosis of stress urinary incontinence in women,which can display the severity of the patient′s disease condition,intuitively and quantitatively reflect the functions of the bladder and urethra,and the more severe disease condition is associated with the lower levels of maximum detrusor pressure,maximum urethral closure pressure and semi-filling pressure.
