早期子宫内膜癌术后复发的危险因素分析

Analysis of Risk Factors for Postoperative Recurrence of Early Endometrial Cancer

目的:分析早期子宫内膜癌术后复发的危险因素。方法:回顾性分析2019年2月—2020年3月于浙江大学医学院附属第一医院行手术治疗的200例早期子宫内膜癌患者的临床资料,根据其3年内复发情况分为复发组与未复发组,采用Logistic回归分析早期子宫内膜癌患者术后复发的危险因素。结果:200例早期子宫内膜癌患者中,复发14例(7.00%)。复发组发病年龄、肿瘤直径大于未复发组,病理分化级别、肿瘤蛋白53(P53)阳性率、Ki-67阳性率、雌激素受体(ER)阳性率、孕激素受体(PR)阳性率、高于未复发组,化疗次数少于未复发组,放疗率低于未复发组,差异有统计学意义(P<0.05)。两组病理类型比较,差异有统计学意义(P=0.001)。Logistic回归分析显示,发病年龄≥60岁、病理分化Ⅱ级、P53阳性、ER阳性是早期子宫内膜癌术后复发的独立危险因素,差异有统计学意义(P<0.05)。结论:发病年龄≥60岁、病理分化Ⅱ级、P53阳性、ER阳性是早期子宫内膜癌术后复发的独立危险因素,需及时对比类患者进行干预,以改善预后。

Objective:To analyze the risk factors for postoperative recurrence of early endometrial cancer.Methods:Clinical data of 200 patients with early endometrial cancer who underwent surgical treatment in the First Affiliated Hospital of Zhejiang University School of Medicine from February 2019 to March 2020 were retrospectively analyzed.According to the recurrence within 3 years,patients were divided into recurrence group and non-recurrence group.The risk factors for postoperative recurrence of patients with early endometrial cancer were analyzed by Logistic regression analysis.Results:Among the 200 patients with early endometrial cancer,14 cases(7.00%)had a recurrence.The age of onset and tumor diameter in the recurrence group were higher than those in the non-recurrence group,and the pathological differentiation grade,tumor protein 53(P53)positive rate,Ki-67 positive rate,estrogen receptor(ER)positive rate,and progesterone receptor(PR)positive rate,in the recurrence group were higher than those in the non-recurrence group,and the number of chemotherapy in the recurrence group was less than that in the non-recurrence group,and the radiotherapy rate in the recurrence group was lower than that in the non-recurrence group,and the difference was statistically significant(P<0.05).There was a statistically significant difference in pathological types between the two groups,and the difference was statistically significant(P=0.001).Logistic regression analysis showed that the age of onset of≥60 years old,pathological differentiation gradeⅡ,P53 positive,and ER positive were independent risk factors for postoperative recurrence of early endometrial cancer,and the difference was statistically significant(P<0.05).Conclusions:The age of onset of≥60 years old,pathological differentiation gradeⅡ,P53 positive,and ER positive are independent risk factors for postoperative recurrence of early endometrial cancer,timely intervention in such patients is needed to improve prognosis.
