二甲双胍联合胰岛素治疗妊娠期糖尿病的临床效果及妊娠结局研究

Clinical effect and pregnancy outcome of metformin combined with insulin in treatment of gestational diabetes mellitus

目的研究妊娠期糖尿病患者应用二甲双胍联合胰岛素治疗的临床效果与妊娠结局。方法选取52例妊娠期糖尿病患者,使用抽签法分成研究组与对照组,各26例。研究组应用二甲双胍联合胰岛素治疗,对照组应用单一胰岛素治疗。比较两组治疗效果。结果治疗后研究组糖化血红蛋白(HbA1c)、空腹血糖(FPG)及餐后2 h血糖(2 h PG)指标均低于对照组(P<0.05)。治疗后研究组的C-反应蛋白(CRP)、脂蛋白相关性磷脂酶A2(Lp-PLA2)、白细胞介素-6(IL-6)及肿瘤坏死因子(TNF-α)指标均低于对照组(P<0.05)。研究组不良妊娠结局发生率3.85%低于对照组的26.92%(P<0.05)。结论二甲双胍联合胰岛素应用于妊娠期糖尿病患者中,可降低不良妊娠结局发生率,同时降低患者的血糖水平,改善炎症水平,值得临床应用。

Objective To study the clinical effect and pregnancy outcome of metformin combined with insulin in patients with gestational diabetes.Methods 52 patients with gestational diabetes mellitus were selected and divided into study group and control group with 26 cases each.The study group was treated with metformin combined with insulin,and the control group was treated with single insulin.The therapeutic effect of the two groups was compared.Results After treatment,the indexes of HbAlc,fasting blood glucose(FPG)and 2 h postprandial blood glucose(2 h PG)in the study group were lower than those in the control group(P<0.05).After treatment,the indexes of C-reactive protein(CRP),lipoprotein-associated phospholipase A2(Lp-PLA2),interleukin-6(IL-6)and tumor necrosis factor(TNF-α)in the study group were lower than those in the control group(P<0.05).The incidence of adverse pregnancy outcome in the study group was 3.85%lower than that in the control group(26.92%)(P<0.05).Conclusion The application of metformin combined with insulin in patients with gestational diabetes can reduce the incidence of adverse pregnancy outcomes,reduce the blood sugar level of patients,and improve the level of inflammation,which is worthy of clinical application.
