经尿道等离子前列腺电切与剜除的效果与安全性的比较研究

Comparative Study on the Efficacy and Safety of Transurethral Plasma Prostatectomy and Enucleation

目的:研究比较经尿道等离子前列腺电切术与经尿道等离子前列腺剜除术治疗的效果与安全性。方法:以我院收治的前列腺增生患者为研究对象,共60例,选取时间范围在2021年4月—2023年3月,根据不同术式进行分组,分为A组(经尿道等离子前列腺电切术治疗)、B组(经尿道等离子前列腺剜除术治疗),各30例。比较两组的治疗效果及安全性。结果:两组患者的手术时间比较高度相似(P>0.05);而B组的术中出血量少于A组,切除腺体量大于A组,卧床时间短于A组,比对差异显著(P<0.05)。两组患者术前、术后1个月的NRS评分、IPSS评分、QOL评分比对具有较高的相似度(P>0.05);但两组患者手术后1个月的各项评分相比术前有不同程度改善,比对差异显著(P<0.05)。B组、A组患者的并发症总发生率比对无明显差异(P>0.05)。结论:在前列腺增生患者的手术治疗中,运用经尿道等离子前列腺电切术或剜除术治疗,两种术式均可改善患者疼痛度及生活质量,两种术式治疗后的并发症无明显差异,但是剜除术的术中出血量更低,切除腺体量大于A组,且卧床时间更短。

Objective:To compare the efficacy and safety of transurethral plasma prostatectomy and transurethral plasma enucleation of the prostate.Methods:A total of 60 patients with benign prostatic hyperplasia admitted to our hospital were selected as the research subjects,with a time range of April 2021 to March 2023.They were divided into group A(treated with transurethral plasma prostatectomy)and group B(treated with transurethral plasma prostatectomy)based on different surgical methods,with 30 patients in each group.Compare the treatment efficacy and safety between the two groups.Results:The surgical time of the two groups of patients was highly similar(P>0.05);However,the intraoperative bleeding in Group B was less than that in Group A,the amount of glandular resection was greater than that in Group A,and the bedtime was shorter than that in Group A,with a significant difference(P<0.05).The comparison of NRS scores,IPSS scores,and QOL scores between the two groups of patients before and 1 month after surgery showed a high degree of similarity(P>0.05);However,the scores of the two groups of patients at one month after surgery showed varying degrees of improvement compared to before surgery,with significant differences(P<0.05).There was no significant difference in the total incidence of complications between Group B and Group A patients(P>0.05).Conclusion:In the surgical treatment of patients with benign prostatic hyperplasia,transurethral plasma prostatectomy or enucleation is used.Both surgical methods can improve the patient’s pain and quality of life.There is no significant difference in complications between the two surgical methods,but enucleation results in lower intraoperative bleeding,larger amount of removed glands than group A,and shorter bed rest time.
