基于急救中心开展血液净化技术对脓毒症治疗的应用研究

摘要 目的:探讨急救中心开展血液净化技术对脓毒症治疗的影响。方法:随机将2021年4月—2023年4月我院急救中心收治的90例脓毒症患者分为三组:CBP组、IBP组、对照组,每组30例。CBP组予以连续性血液净化技术(CBP)治疗,IBP组予以间歇性血液净化技术(IBP)治疗,对照组予以常规对症治疗。比较三组治疗前后急性生理参数、血清炎症指标、血生化指标变化,以及不良反应及治疗情况。结果:较治疗前,治疗后三组T、HR、WBC值均下降,MAP值上升(P<0.05);且三组间参数存在差异(P<0.05):T、HR、WBC值:CBP组<IBP组<对照组,MAP值:CBP组>IBP组>对照组。较治疗前,治疗后三组TNF-α、IL-6、PCT、CRP值均下降(P<0.05);且三组间的数值存在差异(P<0.05):CBP组<IBP组<对照组。较治疗前,治疗后三组Lac、BUN、ALT、PT值均下降(P<0.05);且三组间的数值存在差异(P<0.05):CBP组<IBP组<对照组。三组不良反应发生率无明显差异(P>0.05),治疗情况存在差异(P<0.05):CBP组治疗有效率高于IBP组、对照组。结论:急救中心开展血液净化技术,尤其是CBP,可以改善患者生理参数和生化指标,降低患者炎症程度,提高临床疗效,且安全性较高,值得推广应用。 Objective:To explore the influence of blood purification technology on the treatment of sepsis in emergency centers.Methods:90 patients with sepsis admitted to the emergency center of our hospital from April 2021 to April 2023 were randomly divided into three groups:CBP group,IBP group and control group,with 30 cases in each group.The CBP group was treated with continuous blood purification technology(CBP),the IBP group was treated with intermittent blood purification technology(IBP),and the control group was treated with conventional symptomatic treatment.The changes of acute physiological parameters,serum inflammatory indexes,blood biochemical indexes,adverse reactions and treatment were compared between the three groups before and after treatment.Results:Compared with before treatment,the values of T,HR and WBC were all decreased after treatment,while the values of MAP were increased(P<0.05).Moreover,there were differences in parameters among the three groups(P<0.05):T,HR and WBC values:CBP group<IBP group<control group,MAP value:CBP group>IBP group>control group.Compared with before treatment,the values of TNF-α,IL-6,PCT and CRP in three groups were decreased after treatment(P<0.05).There was a difference between the three groups(P<0.05):CBP group<IBP group<control group.Compared with before treatment,the values of Lac,BUN,ALT and PT in the three groups were decreased after treatment(P<0.05).There was a difference between the three groups(P<0.05):CBP group<IBP group<control group.There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05),but there were differences in treatment conditions(P<0.05):the effective rate of CBP group was higher than that of IBP group and control group.Conclusion:Blood purification technology carried out in emergency centers,especially CBP,can improve the physiological parameters and biochemical indexes of patients,reduce the degree of inflammation in patients,improve clinical efficacy,and have high safety,which is worthy of promotion and application.
