产前不同干预方式对足月孕妇产后泌乳的影响

Effects of different prenatal interventions on postpartum lactation in term pregnant women

目的:探究产前不同干预方式的安全性及对足月孕妇产后泌乳的影响。方法:选取2020年1月~2022年1月合肥市第一人民医院产科住院的194例足月孕妇,最终纳入172例。采用随机数表法将孕妇分为对照组(n=57)、A组(n=58)、B组(n=57)。对照组采用常规干预方式,A组在对照组基础上产前采用七步乳房按摩法,B组在对照组基础上产前采用吸奶器干预,比较3组孕妇分娩结局、泌乳情况、乳腺相关并发症以及新生儿喂养情况。结果:A、B组产后泌乳Ⅱ期启动时间短于对照组(P<0.05),且产后24、48、72 h泌乳量多于对照组(P<0.05)。A、B组6个月纯母乳喂养率高于对照组(P<0.05)。结论:足月孕妇在产前采用七步乳房按摩法以及采用吸奶器进行干预泌乳,较为安全,能够缩短产后的泌乳启动时间,提高泌乳量,进一步提高产后纯母乳喂养率。

Objective:To investigate the safety and impact of different prenatal intervention on postpartum lactation in full-term pregnant women.Methods:172 of the 194 full-term pregnant women admitted to our hospital between January 2020 and January 2022 were eventually included,and divided into control group(n=57),group A(n=58),and group B(n=57)using random number table.Women in the control group underwent conventional intervention,and those in group A received seven-step breast massage before delivery and those in group B were given breast pump intervention before delivery besides the primary intervention as conducted in control group.Then the three groups were compared concerning the delivery outcomes,lactation,breast related complications as well as neonatal feeding status.Results:The starting time of stage II postpartum lactation was shorter in group A and B than in control group(P<0.05),and the postpartum volume of lactation at 24,48 and 72 hour was greater in both group A and B than in the control group(P<0.05).The 6-month exclusive breastfeeding rate in group A and B was higher than that in the control group(P<0.05).Conclusion:Intervention of lactation before delivery using seven-step breast massage and a breast pump can be safer for full-term pregnant women,and this care can shorten the starting time of postpartum lactation,increase lactation volume,and further improve the rate of pure breast feeding after delivery.
