多学科联合诊疗模式融入住院医师规范化培训课程建设的探索

Exploration of multi-disciplinary treatment in standardized training for residents in the nephrology department

目的探讨多学科联合诊疗模式融入住院医师规范化培训课程建设的教学效果。方法以上海市中西医结合医院肾病科住培医师为研究对象,在2021年1月1日—2021年11月30日期间随机选取16人作为对照组,应用传统临床带教方法。在2022年1月1日—2022年11月30日期间随机选取16人作为观察组,进行多学科联合诊疗的教学方法。将2组住培医师的出科考成绩、调查问卷、满意度调查进行比较,观察教学效果。结果观察组出科考成绩高于对照组;在提高自主学习能力、掌握理论知识能力、分析解决问题能力、临床思维能力、团队协作和沟通能力等方面,观察组明显高于对照组;满意度调查显示,观察组满意率较对照组明显升高,差异均具有统计学意义(P<0.05)。结论采用多学科联合诊疗模式融入肾病科住培医师临床带教中具有较好的教学效果,能够提高住培医师的各项临床能力。

Objective To explore the teaching effect of multi-disciplinary treatment(MDT)integrated into the standardized training of residents in the nephrology department.Methods Some resident physicians who received standardized training in the Department of Nephrology of Shanghai TCM-integrated Hospital fell into two groups.Sixteen residents were chosen randomly from Jan.1 st,2021 to Nov.30 th,2021 to form the control group which received clinical training in the traditional way.Another sixteen residents were chosen randomly from Jan.1 st,2022 to Nov.30 th,2022 to form the experimental group which received training by means of MDT.Through comparisons in terms of exit examination scores,and questionnaire and satisfaction survey results,the teaching effects of two groups were observed.Results The scores of the exit examinations of the experimental group were higher than those of the control group.The questionnaire survey results showed that the former group was significantly better than the latter in learner autonomy,theoretical knowledge,problem solving abilities,clinical reasoning,teamwork and communication skills.The satisfaction survey showed that the experimental group was more satisfied with the teaching than control group with statistically significant difference(P<0.05).Conclusion The application of MDT in the standardized training for resident physicians in the nephrology department could yield positive results,improving their clinical competences.
